Расстройства тревожно-депрессивного спектра (ТДР) представляют собой значимую медико-социальную проблему, существенно отягощающую течение соматических заболеваний. Несмотря на понимание влияния аффективных расстройств на течение и лечение хронической лор-патологии, распространенность и структура ТДР, в частности у пациентов с хроническим неспецифическим фарингитом (ХНФ), практически не изучены.</p></sec><sec><title>Цель</title><p>Цель. Оценить распространенность, структуру и выраженность ТДР у пациентов с ХНФ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовано 90 пациентов основной группы с верифицированным диагнозом «хронический фарингит», проходивших амбулаторное и стационарное лечение на базе ГБУЗ НО «НОКБ им. Н.А. Семашко» и кафедры болезней уха, горла и носа им. В.Ю. Шахова ФГБОУ ВО «ПИМУ» МЗ РФ, и 40 добровольцев группы сравнения без хронической воспалительной патологии глотки. Скрининг аффективных нарушений проводился с использованием госпитальной шкалы тревоги и депрессии (HADS), оценивалась структура расстройств (изолированная тревога, изолированная депрессия, смешанное расстройство), фенотип депрессивного синдрома и отдельно – выраженность канцерофобии.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Показано, что общая распространенность ТДР у пациентов с ХНФ составила 38,9%, в их структуре достоверно преобладали изолированное тревожное (57,1%) и смешанное тревожно-депрессивное расстройство (34,3%). Выявлена тенденция к преобладанию тревожного фенотипа депрессии (80,0%) над меланхолическим у пациентов с ХНФ. У 34,4% респондентов с тревожными расстройствами отмечены канцерофобические переживания.</p></sec><sec><title>Выводы</title><p>Выводы. Таким образом, ХНФ ассоциирован с высокой частотой тревожно-депрессивных расстройств, преимущественно тревожного и смешанного спектра. Полученные данные обосновывают необходимость междисциплинарного взаимодействия оториноларингологов и психотерапевтов для оптимизации лечебно-реабилитационных мероприятий.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Anxiety-depressive spectrum disorders (ADSD) represent a significant medico-social burden, substantially complicating the course of somatic diseases. Despite the recognized impact of affective disorders on the course and treatment outcomes of chronic ENT pathology, the prevalence and structure of ADSD, particularly in patients with chronic nonspecific pharyngitis (CNP), remain largely unexplored.</p></sec><sec><title>Aim</title><p>Aim. Тo assess the prevalence, structure and severity of ADSD in patients with CNP.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We screened 90 patients in the treatment group with a verified diagnosis of chronic pharyngitis who underwent outpatient and inpatient treatment at N.A. Semashko Nizhny Novgorod Region Clinical Hospital and Department of Ear, Throat, and Nose Diseases named after V.Yu. Shakhov of Privolzhsky Research Medical University of the Ministry of Health of the Russian Federation and 40 volunteers in the comparison group without chronic inflammatory pharyngeal pathology. Screening for affective disorders was performed using the Hospital Anxiety and Depression Scale (HADS). We evaluated the structure of the disorders (isolated anxiety, isolated depression, mixed disorder), the phenotype of depressive syndrome, and the specific severity of cancerophobia.</p></sec><sec><title>Results</title><p>Results. The overall prevalence of ADSD in patients with CNP was 38.9%. The structure of affective pathology was significantly dominated by isolated anxiety (57.1%) and mixed anxiety-depressive disorder (34.3%). A trend towards a predominance of the anxious depression phenotype (80.0%) over the melancholic phenotype was observed in the CNP group. Cancerophobic experiences were reported by 34.4% of respondents with anxiety disorders.</p></sec><sec><title>Conclusions</title><p>Conclusions. Chronic nonspecific pharyngitis is associated with a high frequency of anxiety-depressive disorders, primarily of the anxious and mixed spectrum.
