Ожирение неблагоприятно влияет на менструальный цикл (МЦ) и наступление беременности. Один из способов лечения ожирения – выполнение бариатрической операции (БО). Самая частая из них – лапароскопическая продольная резекция желудка (ЛПРЖ), однако ее влияние на МЦ изучено недостаточно.</p></sec><sec><title>Цель</title><p>Цель. Оценить МЦ у женщин репродуктивного возраста с ожирением после ЛПРЖ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исходно и через 12 мес. после ЛПРЖ у 47 женщин с ожирением рассчитывали индекс массы тела (ИМТ), индекс инсулинорезистентности HOMA-IR, определялись антимюллеров гормон плазмы (АМГ) и характеристики МЦ, оценивалась длительность ожирения, количество попыток снижения веса более 10%, наличие операций на яичниках и отягощенной наследственности по ранней менопаузе. Пациентки были разделены на группы в зависимости от МЦ до ЛПРЖ (регулярный цикл, n = 24; нарушения МЦ, n = 23), а также стратифицированы по динамике МЦ после операции: восстановление, сохранение или возникновение нарушений впервые.</p></sec><sec><title>Результаты</title><p>Результаты. У всех пациенток через 12 мес. после ЛПРЖ ИМТ снизился, показатели углеводного обмена улучшились. У 48,9% пациенток отмечались нарушения МЦ до БО, и лишь в 56,5% случаев отмечалась нормализация менструального цикла через 12 мес. после операции. У 47,8% пациенток после ЛПРЖ были зафиксированы различные нарушения МЦ, и у 52% женщин возникли впервые после операции. Наличие нарушения МЦ после ЛПРЖ имело связь с ИМТ, динамикой снижения веса, выраженностью инсулинорезистентности и уровнем АМГ. Отягощенная наследственность по ранней или преждевременной менопаузе повышала вероятность развития нарушения МЦ после ЛПРЖ.</p></sec><sec><title>Выводы</title><p>Выводы. ЛПРЖ неоднозначно влияет на МЦ у женщин с ожирением. Несмотря на улучшение массы тела и углеводного обмена, у 52% пациенток с исходно регулярным циклом после операции впервые возникли нарушения МЦ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Obesity adversely affects the menstrual cycle (MC) and the onset of pregnancy. One of the ways to treat obesity is to perform bariatric surgery (BS). The most common of these is laparoscopic sleeve gastrectomy (LSG), but its effect on MC has not been sufficiently studied.</p></sec><sec><title>Aim</title><p>Aim. To evaluate MC in obese women of reproductive age after LSG.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Initially and 12 months after LSG for 47 obese women was calculated body mass index (BMI), HOMA-IR insulin resistance index, plasma anti-muller hormone (AMH) and MC characteristics, determined the duration of obesity, the number of weight loss attempts of more than 10%, the presence of ovarian surgery and burdened heredity by early menopause. The patients were divided into groups depending on their MC before LSG (regular cycle, n = 24; irregular MC (IMC), n = 23), and also stratified by the dynamics of MC after surgery: recovery, preservation or occurrence of disorders for the first time.</p></sec><sec><title>Results</title><p>Results. BMI decreased in all patients 12 months after LSG, and carbohydrate metabolism improved. 48.9% of patients had irregular MC before surgery, and only 56.5% of cases showed normalization of the menstrual cycle 12 months after surgery. 47.8% of patients had irregular MC after LSG, and 52% of women had irregular MC for the first time after surgery. The presence of irregular MC after LSG was associated with BMI, weight loss dynamics, severity of insulin resistance, and AMH levels. Burdened heredity due to early or premature menopause increased the likelihood of developing irregular MC after LSG.</p></sec><sec><title>Conclusions</title><p>Conclusions. LSG has an ambiguous effect on MC in obese women.
