Хроническая болезнь почек (ХБП) – широко распространенное неинфекционное хроническое заболевание, приводящее к резкому ухудшению качества жизни, высокой смертности среди пациентов. Гиперфосфатемия – независимый фактор повышения общей и сердечно-сосудистой смертности на всех стадиях ХБП.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность применения бескальциевых фосфат-связывающих препаратов (севеламера карбоната и комплекса бета-железа (III) оксигидроксида сахарозы и крахмала) в сравнительном аспекте у пациентов, получающих программный гемодиализ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен ретроспективный анализ 94 медицинских карт пациентов с терминальной стадией ХБП, получающих заместительную почечную терапию. Критерии включения: пациенты в возрасте от 18 до 85 лет с терминальной стадией ХБП, находящиеся на программном гемодиализе на протяжении 12 мес. и получающие фосфат-связывающие препараты.</p></sec><sec><title>Результаты</title><p>Результаты. В общей группе больных (94 человека) в динамике годового наблюдения содержания уровня фосфора отмечались статистически значимые изменения (p = 0,001). В первой группе пациентов, принимающих комплекс бета-железа (III) оксигидроксида сахарозы и крахмала, выявлено статистически значимое снижение уровня фосфора в динамике (p = 0,046), во второй группе пациентов, получающих севеламера карбонат, также было выявлено такое статистически значимое изменение (p &lt; 0,001). В оценках использовался критерий Фридмана. При оценке дополнительных эффектов от приема фосфат-связывающих препаратов по двум группам исследуемых было выявлено наличие статистически значимого повышения уровня альбумина крови (p &lt; 0,001), повышение уровня трансферрина (p &lt; 0,001). Статистически значимое снижение уровня холестерина было выявлено в группе пациентов, принимающих севеламера карбонат (p = 0,044). В оценках использовался критерий Фридмана.</p></sec><sec><title>Выводы</title><p>Выводы. Проведенные нами исследования эффективности фосфат-связывающей терапии показали, что в обеих группах наблюдались позитивные изменения по нормализации уровня фосфора в крови на протяжении всего периода, которые были статистически достоверны в обеих группах сравнения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Chronic kidney disease (CKD) is a widespread, non-communicable chronic condition that leads to a significant deterioration in quality of life and high mortality among patients. Hyperphosphatemia is an independent factor in increasing all-cause and cardiovascular mortality at all stages of CKD.</p></sec><sec><title>Aim</title><p>Aim. To comparatively evaluate the efficacy of calcium-free phosphate binders (sevelamer carbonate and beta-iron(III) oxyhydroxide complex of sucrose and starch) in patients receiving hemodialysis.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective analysis of 94 medical records of patients with end-stage CKD receiving renal replacement therapy was conducted. Inclusion criteria: Patients aged 18 to 85 years with end-stage CKD, undergoing hemodialysis for 12 months and receiving phosphate binders.</p></sec><sec><title>Results</title><p>Results. In the total group of patients (94 people), statistically significant changes in phosphorus levels were observed over the course of the year-long observation period (p = 0.001). In the first group of patients taking beta-iron (III) sucrose oxyhydroxide starch complex, a statistically significant decrease in phosphorus levels over time (p = 0.046) (Friedman test used). In the second group of patients receiving sevelamer carbonate, a statistically significant change was also observed (p &lt; 0.001) (Friedman test used). When assessing the additional effects of phosphate binders, statistically significant changes were found across the two study drug groups: increased blood albumin levels (p &lt; 0.001) and increased transferrin levels (p &lt; 0.001). Statistically significant changes in cholesterol reduction were found in the sevelamer carbonate group (p = 0.044). The Friedman test was used in the assessments. No statistically significant changes were found in C-reactive protein or ferritin levels in the two study groups.</p></sec><sec><title>Conclusion</title><p>Conclusion.
