Комбинированная длительная кислородотерапия (ДКТ) – это наиболее оптимальный режим ДКТ в домашних условиях у пациентов с хронической дыхательной недостаточностью (ХДН). Она характеризуется использованием стационарных форм подачи кислорода в покое и во время сна и портативных приборов кислородотерапии при активной деятельности. Цель. Оценить эффективность и безопасность применения портативного кислородного концентратора пациентов с ХДН.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 15 пациентов с ХДН. В ходе исследования была проведена оценка клинического состояния пациентов, показателей газового состава крови, функциональных характеристик дыхательной системы, легочной гемодинамики и переносимости физической нагрузки на момент включения в исследование, а также через 3, 6, 9 и 12 мес. использования портативного кислородного концентратора. Оценка физической активности проводилась путем определения количества пройденных шагов в условиях без кислородной поддержки и при использовании портативного кислородного концентратора, данные были получены из фитнес-браслетов пациентов на протяжение 24 ч в сут.</p></sec><sec><title>Результаты</title><p>Результаты. На фоне длительной кислородотерапии через 12 мес. отмечалось улучшение клинической симптоматики: снижение одышки по шкале Borg с 6 до 3 баллов (р = 0,001), MRC с 3 до 2 баллов (p = 0,035), ВАШ с 7 до 5 баллов (p &lt; 0,001), улучшение показателей газообмена SpO2 с 92 до 94% (p = 0,002), стабилизации функциональных показателей: диффузионной способности легких c 37 до 35% (p = 1,000) и снижение систолического давления в легочной артерии c 55 до 50 мм рт. ст. Отмечалось достоверное улучшение физической активности: количество пройденных шагов с 82,5 увеличилось до 360 (р &lt; 0,001).</p></sec><sec><title>Выводы</title><p>Выводы. Полученные данные подтверждают клиническую эффективность и безопасность применения портативных кислородных концентраторов в рамках длительной кислородной терапии у пациентов с ХДН.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The combined long-term oxygen therapy (COT) is the most optimal home-based COT regimen for patients with chronic respiratory failure (CRF). It is characterized by the use of stationary oxygen delivery systems during seated rest and sleep and portable oxygen therapy devices during active activities.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the efficacy and safety of portable oxygen concentrators in patients with chronic respiratory failure.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 15 patients with chronic respiratory failure. The study assessed the patients' clinical condition, blood gas parameters, respiratory system function, pulmonary hemodynamics, and exercise tolerance at entrollment, and 3, 6, 9 and 12 months using the portable oxygen concentrator. Physical activity was assessed by measuring the number of steps walked without oxygen support and with a portable oxygen concentrator. Data were collected from patients' fitness trackers for 24 hours a day.</p></sec><sec><title>Results</title><p>Results. After 12 months of long-term oxygen therapy, the patients showed the following improvements in clinical symptoms: a decrease in the BORG dyspnea scores from 6 to 3 (p = 0.001), MRC scores from 3 to 2 (p = 0.035), and VAS scores from 7 to 5 (p &lt; 0.001); an improvement in SpO2 gas exchange parameters from 92 to 94% (p = 0.002); stabilization of functional parameters: diffusing capacity of the lungs from 37 to 35% (p = 1.000) and a decrease in the pulmonary artery systolic pressure from 55 to 50 mmHg. The patients showed significant improvements in physical activity: the number of steps walked increased from 82.5 to 360 (p &lt; 0.001).</p></sec><sec><title>Conclusions</title><p>Conclusions.
