Муковисцидоз остается одним из наиболее значимых наследственных мультисистемных заболеваний детского возраста, для которого характерны прогрессирующее поражение бронхолегочной системы, внешнесекреторная недостаточность поджелудочной железы и выраженная вариабельность клинического фенотипа в зависимости от генотипа и сроков начала патогенетической терапии. В последние годы появление CFTR-модуляторов принципиально изменило терапевтическую парадигму, поскольку впервые стало возможным воздействовать на молекулярный дефект, лежащий в основе заболевания, а не только на его клинические проявления.</p></sec><sec><title>Цель</title><p>Цель. Проанализировать региональную структуру популяции пациентов с муковисцидозом в Южном федеральном округе (ЮФО) и оценить охват детской когорты больных таргетной терапией.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен описательный анализ региональных данных о пациентах с муковисцидозом в ЮФО. Оценивались численность пациентов, их распределение по субъектам округа, число впервые выявленных случаев в 2025 г., объем передачи пациентов в терапевтическое звено, обеспеченность CFTR-модуляторами и структура назначаемых препаратов.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что в ЮФО зарегистрирован 521 пациент с муковисцидозом, включая 366 детей и 155 взрослых. Крупнейшие когорты сосредоточены в Краснодарском крае, Ростовской области, Республике Крым и Волгоградской области. В 2025 г. в округе диагностировано 20 новых случаев заболевания при ожидаемом числе 25–30, рассчитанном исходя из средней частоты муковисцидоза в Российской Федерации. Таргетную терапию получают 246 детей, тогда как 120 пациентов остаются без патогенетически ориентированного лечения. Среди таргетных препаратов доминирует тройная комбинация ивакафтор + тезакафтор + элексакафтор и ивакафтор, представленная на российском рынке под торговыми наименованиями Трилекса® (Tuteur S.A.C.I.F.I.A., Аргентина) (78%) и Трикафта® (Vertex Pharmaceuticals) (15,9%).</p></sec><sec><title>Выводы</title><p>Выводы. Полученные данные свидетельствуют о существенном прогрессе в доступности патогенетически ориентированной терапии в ЮФО, однако сохраняются генетические, возрастные и организационные барьеры, ограничивающие полный охват пациентов высокоэффективными CFTR-модуляторами. Региональный анализ подчеркивает необходимость дальнейшего совершенствования неонатального скрининга, расширения генотип-ориентированной таргетной терапии и обеспечения преемственности помощи подросткам и молодым взрослым.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cystic fibrosis (CF) remains one of the most significant inherited multisystem diseases of childhood, characterized by progressive damage to the bronchopulmonary system, exocrine pancreatic insufficiency, and marked clinical phenotype variability governed by both underlying genotype and the timing of pathogenetic therapy initiation. Recently, the advent of CFTR-modulators dramatically changed the paradigm of CF management, as it has become possible for the first time to address the molecular defect underlying the disease, rather than just its clinical manifestations.</p></sec><sec><title>Aim</title><p>Aim. To analyze the regional structure of the CF patient population in the Southern Federal District (SFD) and assess the coverage of the pediatric cohort with targeted therapy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A descriptive analysis was conducted using the regional data on CF patients in the SFD. The following parameters were assessed: the number of patients, patient distribution across the SFD entities, the number of newly diagnosed cases in 2025, the volume of patient referrals to the therapeutic facilities, availability of CFTR-modulators, and prescription profiles.</p></sec><sec><title>Results</title><p>Results. The SFD CF population was estimated at 521 patients, with 366 children and 155 adults. The Krasnodar Territory, Rostov Region, Republic of Crimea, and Volgograd Region had the largest cohorts of CF patients. In 2025, 20 new cases of the disease were diagnosed in the district, as compared with the expected number of 25–30, calculated from population-based data on the average incidence of CF in the Russian Federation. 246 children received targeted therapy, while 120 patients remained without pathogen-oriented treatment. Among targeted drugs dominated the triple combination composed of elexacaftor, tezacaftor, and ivacaftor introduced to the Russian market under the trade name Trilexa® (Tuteur S.A.C.I.F.I.A., Argentina) (78%) and Trikafta® (Vertex Pharmaceuticals) (15.9%).</p></sec><sec><title>Conclusions</title><p>Conclusions. The available data show a signficant progress in the availability of pathogen-oriented therapy in the SFD, but there remain genetic, age-related, and institutional barriers limiting full patient coverage with highly effective CFTR-modulators.
