Муковисцидоз – наследственное заболевание, ассоциированное с прогрессирующим поражением бронхолегочной системы и риском инвалидизации. Внедрение тройных CFTR-модуляторов (ивакафтор / тезакафтор / элексафтор и ивакафтор) существенно улучшило прогноз, однако, высокая стоимость оригинального препарата Трикафта® и ограниченные ресурсы здравоохранения обусловливают интерес к биоэквивалентному генерическому препарату Трилекса®.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность и безопасность применения лекарственных препаратов патогенетической терапии Трикафта и Трилекса у детей с муковисцидозом в Чеченской Республике, а также перехода с оригинального на генерический в рамках одного международного непатентованного наименования (МНН) (ивакафтор + тезакафтор + элексакафтор и ивакафтор), в условиях рутинной клинической практики.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено ретроспективное одноцентровое наблюдательное исследование (Чеченская Республика). В анализ включены 22 пациента в возрасте до 18 лет с генотипом, соответствующим показаниям к тройной CFTR-модуляторной терапии. Сформированы 3 группы: 1-я (n = 5) – терапия только Трикафтой; 2-я (n = 2) – терапия только Трилексой; 3-я (n = 15) – переведенные с Трикафты на Трилексу. Оценивали динамику концентрации хлоридов пота, индекса массы тела (ИМТ), показателей функции внешнего дыхания (ОФВ₁, ФЖЕЛ), частоты легочных обострений и профиль безопасности.</p></sec><sec><title>Результаты</title><p>Результаты. Во всех группах отмечено снижение концентрации хлоридов пота, свидетельствующее о восстановлении функции CFTR. В 3-й группе хлориды пота снизились с 91,0 до 41,0 ммоль/л на фоне Трикафты и оставались стабильными после перехода на Трилексу. ИМТ достоверно увеличился с 14,9 до 15,7 кг/м² на этапе Трикафты (p = 0,002) и продолжал расти после перевода на Трилексу. Показатели ОФВ₁ и ФЖЕЛ оставались стабильно высокими после смены препарата (p &gt; 0,05). Частота легочных обострений снизилась и оставалась минимальной на фоне Трилексы.</p></sec><sec><title>Заключение</title><p>Заключение. Применение как оригинального препарата, так и генерического Трилекса у детей с МВ сопровождается клиническим улучшением ключевых показателей эффективности и благоприятным профилем безопасности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cystic fibrosis is a hereditary disease associated with progressive damage to the bronchopulmonary system and the risk of disability. The introduction of triple CFTR modulators (ivacaftor/tezacaftor/elexaftor and ivacaftor) has significantly improved the prognosis. However, the high cost of the original drug Trikafta® and limited healthcare resources have driven interest in the bioequivalent generic drug Trilexa®.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the efficacy and safety of the pathogenetic therapy drugs Trikafta and Trilexa in children with cystic fibrosis in the Chechen Republic, as well as the transition from the original to the generic version within the same INN (ivacaftor + tezacaftor + elexacaftor and ivacaftor), in routine clinical practice.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective, single-center observational study was conducted in the Chechen Republic. The analysis included 22 patients under 18 years of age with a genotype corresponding to the indications for triple CFTR modulator therapy. Three groups were formed: Group 1 (n = 5) – treatment with Trikafta only; Group 2 (n = 2) – treatment with Trilexa only; Group 3 (n = 15) – patients switched from Trikafta to Trilexa. Changes in sweat chloride concentrations, body mass index (BMI), pulmonary function parameters (FEV₁, FVC), the frequency of pulmonary exacerbations, and the safety profile were assessed.</p></sec><sec><title>Results</title><p>Results. A decrease in sweat chloride concentrations was noted in all groups, indicating restoration of CFTR function. In Group 3, sweat chloride levels decreased from 91.0 to 41.0 mmol/L while on Trikafta and remained stable after switching to Trilexa. BMI significantly increased from 14.9 to 15.7 kg/m² during the Trikafta phase (p = 0.002) and continued to increase after switching to Trilexa. FEV₁ and FVC remained consistently high after switching (p &gt; 0.05). The incidence of pulmonary exacerbations decreased and remained minimal while on Trilexa.</p></sec><sec><title>Conclusion</title><p>Conclusion.
