Риск малигнизации предраковой патологии толстого кишечника напрямую зависит от размеров выявленных при колоноскопии новообразований. Так, по данным различных авторов, новообразования диаметром до 2,0 см характеризуются риском выявления злокачественных клеток, составляющим 12,5%. Если размеры новообразования более 2,0 см, риск малигнизации существенно возрастает и, по данным различных авторов, колеблется от 31 до 57%. Субъективная оценка эндоскопистом размера новообразований часто не соответствует их реальной величине, что может приводить к неправильному выбору лечебной тактики. Цель. Оценить эффективность подсистемы аппаратного измерения размеров новообразований в просвете толстого кишечника (AI-assisted neoplasm size estimation, AIANSE) при выполнении колоноскопии с использованием системы искусственного интеллекта для детекции новообразований толстого кишечника (AI-assisted colonoscopy, AIAC).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С 2022 по 2024 г. в ГАУЗ «Челябинский областной клинический центр онкологии и ядерной медицины» совместно с российской компанией EVA Lab была разработана и внедрена в клиническую практику система поддержки принятия врачебных решений на основе алгоритмов искусственного интеллекта. Всего в исследование было включено 1 763 пациента, у которых было размечено около 27 000 кадров.</p></sec><sec><title>Результаты</title><p>Результаты. Компьютерная обработка эндоскопического изображения продемонстрировала эффективность как при детекции новообразований, так и при оценке их размеров. Это подтверждается сопоставлением данных обработки изображений при колоноскопии с реальными размерами новообразований, определенными после хирургического лечения. Данная система обладает способностью к самообучению, что подтверждается повышением в период с 2022 по 2024 г. показателей специфичности (с 63 до 70%), чувствительности (с 64 до 70%) и точности (с 66 до 71%).</p></sec><sec><title>Выводы</title><p>Выводы. Впервые создана система компьютерного анализа и измерения размеров новообразования AIANSE, которая продемонстрировала эффективность при оценке размеров новообразований: точность определения составила 71%. Наибольшая эффективность отмечена при измерении новообразований диаметром 5 мм: точность составила 76%.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The risk of malignancy of precancerous colonic lesions directly depends on the size of the neoplasms detected during colonoscopy. According to various authors, neoplasms up to 2.0 cm in diameter have a 12.5% risk of harboring malignant cells. If the neoplasm is larger than 2.0 cm, the risk of malignancy increases significantly and, according to various authors, ranges from 31 to 57%. The endoscopist’s subjective assessment of neoplasm size often does not correspond to the actual size, leading to inappropriate treatment decisions.</p></sec><sec><title>Aim</title><p>Aim. To develop and evaluate the effectiveness of a hardware-based subsystem for measuring the size of colonic neoplasms (AI-assisted neoplasm size estimation, AIANSE) during colonoscopy with AI-assisted colonoscopy detection (AIAC).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From 2022 to 2024, the Chelyabinsk Regional Clinical Center of Oncology and Nuclear Medicine, in collaboration with the Russian company EVA Lab, developed and implemented a clinical decision support system based on artificial intelligence algorithms. The study included 1,763 patients, from whom approximately 27,000 frames were labeled. Results. Computer processing of endoscopic images demonstrated its effectiveness in both tumor detection and tumor size estimation. This is confirmed by comparing colonoscopy image analysis data with the actual dimensions of the neoplasms determined after surgical treatment. The system possesses self-learning capabilities, as evidenced by the increase in specificity (from 63% to 70%), sensitivity (from 64% to 70%), and accuracy (from 66% to 71%) between 2022 and 2024.</p></sec><sec><title>Conclusions</title><p>Conclusions. AIANSE, a computer-based tumor analysis and size measurement system, was developed for the first time. This system demonstrated its effectiveness in tumor size estimation, with an accuracy of 71%.
