Однако НПВП могут вызывать серьезные осложнения со стороны ЖКТ и сердечно-сосудистой системы. Фармакологи неоднократно предпринимали попытки создать «улучшенные» НПВП, не оказывающие повреждающего действия на ЖКТ. К таким препаратам относятся коксибы, НПВП в комбинации с гастропротекторами (мизопростол, Н2-блокаторы, ИПП), ЦОГ-2/ЛОГ-2 ингибиторы, CINOD. Все эти препараты имеют свои недостатки, некоторые из них используются в практике, другие остались на уровне научных разработок или уже вышли из клинической практики. Амтолметин гуацил - новый «улучшенный» НПВП, созданный на основе неселективного НПВП толметина и обладающий комплексом гастропротективных свойств. Главным из них является повышение концентрации NO в слизистой ЖКТ. В статье рассмотрен механизм действия нового препарата, представлены данные его лабораторных и клинических испытаний.</p></abstract><trans-abstract xml:lang="en"><p>NSAID is a key element in the treatment of acute pain and management of chronic pain. However, NSAIDs may cause serious complications in the gastrointestinal tract and the cardiovascular system. Pharmacologists have been trying to develop an "advanced" NSAID which does not affect the gastrointestinal tract, including coxibs, NSAIDs in combination with gastroprotective agents (misoprostol, H2 blockers, PPI), COX-2/LOX-2 inhibitors, and CINOD. All of these drugs have their limitations, some of them are being used, while others remained in the research phase or were ousted from clinical practice. Amtolmetin guacil is a new "advanced" NSAID based on the non-selective NSAID tolmetin, which posesses a number of gastroprotective attributes. It is specific for being able to increase the NO concentration in the gastrointestinal mucosa.
