Клинически ОА проявляется болью механического характера, припухлостью и непродолжительной утренней скованностью. Согласно эпидемиологическим исследованиям, у лиц, испытывающих хроническую боль, около трети хронической умеренной и сильной боли приходится именно на случаи ОА. В настоящее время рассматривается мультифакторная модель ОА, согласно которой это заболевание является результатом взаимодействия различных генетических, биологических, механических и обменных факторов. По современным представлениям, воспалительные реакции при ОА играют ведущую роль в развитии и прогрессировании этого заболевания. У пациентов с ОА при гистологических исследованиях синовиальной оболочки пораженного сустава выявляется целый ряд изменений, свойственный хроническому воспалению. В сыворотке крови, синовиальной жидкости и в тканях сустава может определяться повышение уровня некоторых провоспалительных цитокинов и снижение противовоспалительных цитокинов, а также развитие адаптивных иммунных клеточных реакций в тканях сустава, наблюдаемых и при других ревматических заболеваниях.</p><p>Благодаря высокой эффективности нестероидные противовоспалительные препараты (НПВП) заняли прочное место в терапии ОА. Во всех современных руководствах НПВП являются препаратами выбора уже на самых первых этапах лечения ОА в виде локальных или пероральных форм. Эторикоксиб является селективным ингибитором циклооксигеназы 2 (ЦОГ-2) и применяется для лечения ОА и других ревматических заболеваний. В статье обсуждаются возможности применения эторикоксиба у лиц с ОА. Представлен обзор клинических исследований эффективности и безопасности эторикоксиба у больных ОА.</p></abstract><trans-abstract xml:lang="en"><p>Osteoarthritis (OA) is one of the most common diseases of the musculoskeletal system.</p><p>The main symptoms of osteoarthritis are pain, stiffness, joint swelling. According to epidemiological studies, about one-third of the chronic moderate and severe pain is associated with OA. Currently, OA is considered as a multifactorial disease resulting from the interaction of various genetic, biological, mechanical, and metabolic factors. Inflammation plays a central role in development and progression of ОА. In patients with OA, histological studies of the synovial membrane detected signs of chronic inflammation. The level of some proinflammatory cytokines may increased and the level of anti-inflammatory cytokines may decreased in blood serum, synovial fluid and joint tissue. Also, adaptive immune cell responses are detected in the joint tissues in patients of OA. Due to its high effectiveness, non-steroidal anti-inflammatory drugs (NSAIDs) are widely used in OA. Oral or local forms of NSAIDs are the drugs of choice in the initial stages of therapy in all guidelines. Etoricoxib is a selective inhibitor of cyclooxygenase 2 (COX-2) and is used for the treatment of OA and other rheumatic diseases.
