Высокая частота развития неопластических процессов шейки матки (ШМ) объясняется увеличением возраста беременных женщин и ростом распространенности папилломавирусной инфекции (ПВИ), частота которой регистрируется на уровне 20% как до, так и после беременности.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить распространенность папилломавирусной инфекции, частоту и структуру заболеваний шейки матки у беременных женщин и определить факторы риска их развития.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен скрининг на ВПЧ среди 2 620 беременных женщин, из них у 682 (29%) обнаружен ВПЧ. 182 беременных с ВПЧ и 148 без ВПЧ были включены в программу дополнительного исследования и были разделены на группы в зависимости от наличия или отсутствия заболеваний шейки матки (ЗШМ). Применялись методы исследования: общеклинические, тест «Квант-21» для определения ВПЧ и ИППП, микроскопия отделяемого влагалища, определение состава микробиоты влагалища методом Фемофлор-16, цитологическое исследование, расширенная кольпоскопия. Использовали метод логит-регрессионных моделей для выявления факторов риска.</p></sec><sec><title>Результаты</title><p>Результаты. Распространенность ЗШМ у носительниц ВПЧ составила 89,4%. Воспалительные ЗШМ имели 72,1% из них по сравнению с 21,4% у пациенток без ВПЧ (р &lt; 0,0001). Частота интраэпителиальных поражений ШМ составила 17,4 против 2,6% соответственно (р = 0,005). Факторами риска развития ЗШМ у беременных женщин, кроме инфицирования ВПЧ высокого канцерогенного риска, наиболее значимыми являются: отсутствие регулярного скрининга РШМ до наступления беременности (ОШ = 34,8), бактериальный вагиноз в анамнезе (ОШ = 15,9), практика прерванного полового акта при сексуальных контактах (ОШ = 14,6). Факторами, свидетельствующими о низком риске указанных заболеваний, являются: участие в ежегодном скрининге РШМ, стабильные партнерские отношения (один половой партнер, замужество), возраст моложе 25 лет и использование презервативов (ОШ менее 0,25).</p></sec><sec><title>Заключение</title><p>Заключение. Распространенность ЗШМ у беременных на фоне ВПЧ-инфекции является высокой, преимущественно воспалительного характера и связана с определенными социально-демографическими и клинико-анамнестическими факторами риска.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. To study the prevalence of human papillomavirus infection, the frequency and structure of cervical diseases in pregnant women and determine the risk factors for their development.</p></sec><sec><title>Material and methods</title><p>Material and methods. Screening for HPV was carried out among 2620 pregnant women. Of these, 682 (29%) had HPV. 182 pregnant women with HPV and 148 without HPV were included in the additional study program and were divided into groups depending on the presence or absence of cervical disease (CWD). The research methods were used: general clinical, the Kvant-21 test to determine HPV and STIs, microscopy of the vaginal discharge, determination of the composition of the vaginal microbiota by the Femoflor 16 method, cytological examination, extended colposcopy. We used the method of logit-regression models to identify risk factors.</p></sec><sec><title>Results</title><p>Results. The prevalence of diseases of the cervix (DC) in HPV carriers was 89.4%. 72.1% of them had inflammatory DC compared to 21.4% in patients without HPV (p &lt; 0.0001). The incidence of intraepithelial lesions was 17.4% versus 2.6%, respectively (p = 0.005). The most significant risk factors for developing DC in pregnant women, in addition to HPV infection of high carcinogenic risk, are: Lack of regular screening for cervical cancer before pregnancy (OR = 34.8), a history of bacterial vaginosis (OR = 15.9), the practice of interrupted sexual intercourse during sexual contacts (OR = 14.6). Factors indicating a low risk of these diseases are: participation in the annual screening of cervical cancer, stable partnerships (one sexual partner, marriage), age under 25, and condom use (OR less than 0.25).</p></sec><sec><title>Conclusion</title><p>Conclusion.
