Апиксабан  – прямой пероральный антикоагулянт, назначаемый для  лечения и  профилактики венозных тромбоэмболий, а также профилактики инсульта у пациентов с неклапанной фибрилляцией предсердий. Прямые оральные антикоагулянты, такие как апиксабан, быстро заменяют терапию варфарином из-за улучшенного профиля эффективности и безопасности, показанного в клинических испытаниях. Однако антикоагулянты могут вызывать серьезные и неблагоприятные лекарственные реакции.</p></sec><sec><title>Цель работы</title><p>Цель работы. Изучение влияния носительства полиморфизмов генов CYP3A4*22 (rs35599367) C &gt; T, CYP3A5*3 (rs776746) A &gt; G, ABCB1 (rs4148738) C &gt; T и ABCB1 (rs1045642) C &gt; T на изменение протромбинового времени (ПТВ) и активированного частичного тромбопластинового времени у пациентов, принимающих апиксабан.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективное обсервационное исследование было включено 36 пациентов. Все пациенты получали апиксабан в дозах в соответствии с инструкцией по применению препарата. У 26 пациентов была диагностирована фибрилляция предсердий, а у 23 пациентов – вторичные тромботические осложнения. Каждому пациенту был произведен забор венозной крови на 4–5-й день с момента назначения апиксабана с целью проведения фармакогенетического тестирования, а также для измерения показателей АЧТВ и ПТВ. Для отбора генов-кандидатов в исследование использовали базу данных PharmGKB. Генотипирование проводилось с помощью методики полимеразной цепной реакции в режиме реального времени. Статистический анализ результатов проводился с помощью программы IBM SPSS Statistics 23.0</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. В нашем исследовании мы оценили влияние полиморфизмов генов ABCB1 (rs1045738) C &gt; T, ABCB1 (rs4148642) C &gt; T на показатели АЧТВ и ПТВ. Статистически значимых ассоциаций обнаружено не было.</p></sec><sec><title>Заключение</title><p>Заключение. Значения ПТВ и АЧТВ у пациентов, принимающих апиксабан, в группах с различными полиморфизмами генов ABCB1 (rs1045738) C &gt; T, ABCB1 (rs4148642) C &gt; T статистически значимо не различались. Необходимы дальнейшие исследования влияния фармакогенетики на профиль безопасности и эффективность апиксабана.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Apixaban is a direct oral anticoagulant prescribed for treatment and prevention of venous thromboembolism (VTE) and for prevention of stroke in patients with nonvalvular atrial fibrillation. Direct oral anticoagulants (DOACs) such as apixaban are rapidly replacing warfarin therapy due to improved efficacy and safety profile shown in clinical trials. However, anticoagulants can cause serious and adverse drug reactions (ADRs).</p></sec><sec><title>Aim</title><p>Aim. Study of the effect of carriage of polymorphisms in CYP3A4*22 (rs35599367) C&gt;T, CYP3A5*3 (rs776746) A&gt;G, ABCB1 (rs4148738) C&gt;T and ABCB1 (rs1045642) C&gt;T genes on the change in prothrombin time (PT) and activated partial thromboplastin time (aPTT) in patients using apixaban.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A total of 36 patients were enrolled onto this prospective observational study. All patients received apixaban at doses in accordance with instructions on how to administer the drug. Atrial fibrillation was diagnosed in 26 patients, and secondary thrombotic complications in 23 patients. To perform pharmacogenetic tests and measure aPTT and PT parameters, venous blood samples from each patient were drawn on Days 4-5 after prescription of apixaban. The PharmGKB database was used to select candidate genes for the study. Genotyping was performed using the real-time polymerase chain reaction technique. Statistical analysis: data were analyzed by using IBM SPSS Statistics program, Version 23.0.</p></sec><sec><title>Results</title><p>Results. In our study, we assessed the effect of polymorphisms in ABCB1 (rs1045738) C&gt;T, ABCB1 (rs4148642) C&gt;T genes on the aPTT and PT parameters. No statistically significant associations were found.</p></sec><sec><title>Conclusion</title><p>Conclusion. The differences between PT and APTT values in patients using apixaban in the groups with different polymorphisms in ABCB1 (rs1045738) C&gt;T, ABCB1 (rs4148642) C&gt;T gene were not statistically significant.
