На сегодняшний день существует большое количество теорий патогенеза полипозного синусита. Активно обсуждается участие Stарhуlососсus аurеus (S. аurеus) в возникновении данной патогенетической реакции.</p></sec><sec><title>Цель</title><p>Цель. Повысить эффективность лечения полипозного синусита на основе исследования влияния персистирующей стафилококковой инфекции на возникновение и течение заболевания.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Авторами было обследовано 105 пациентов с рецидивирующим полипозным синуситом в возрасте от 20 до 65 лет. Всем пациентам при первичном обращении проводили FЕSS – операции по поводу рецидива полипозного синусита. В послеоперационном периоде все больные получали лечение по одной из двух схем: в первой присутствовали антигистаминные средства и топические стероиды, во вторую дополнительно включали кларитромицин. Пациентам были проведены: бактериологическое, микологическое исследования мазков на флору и грибы из полости; цитологическое исследование мазков-отпечатков со слизистой оболочки полости носа (выполняли подсчет нейтрофилов, эозинофилов и лимфоцитов в препарате); иммунологическое и аллергологическое обследования (кожные тесты, уровни специфического IgЕ сыворотки).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Установлено, что наличие у пациента носительства S. аurеus и сенсибилизации к нему, особенно в сочетании с атопией, имеет своим следствием более тяжелое течение полипозного синусита (выраженная назальная обструкция: 91% против 69,5% соответственно) и характеризуется более выраженным эозинофильным воспалением, чем полипозный синусит, ассоциированный с инфицированием прочими микроорганизмами (количество эозинофилов в крови: 7,5 и 4,3% соответственно, уровень эозинофилов в мазках со слизистой оболочки полости носа: 13 и 4,8% соответственно; средний уровень эндотелиальных клеток-предшественников (ЭКП) 17,6 и 8,1 мкг/мл соответственно).</p></sec><sec><title>Выводы</title><p>Выводы. Ведущим методом профилактики рецидивов полипозного синусита по-прежнему остается применение топических глюкокортикостероидов, однако при колонизации в полости носа S. аurеus, помимо базисной терапии топическими глюкокортикостероидами и антигистаминными препаратами, целесообразно применение курса антибактериальной терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Today, there are lot of theories of pathogenesis of polypoid sinusitis. The contribution of Starchylocossus aureus (S. aureus) to the occurrence of this pathogenetic reaction is being actively discussed.</p></sec><sec><title>Objective</title><p>Objective: to improve the effectiveness of treatment of polypoid sinusitis using the results of the study of the effect of persistent staphylococcal infection on the onset and course of the disease.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The authors examined 105 patients with recurrent polyposis sinusitis aged 20 to 65 years. All patients underwent FESS surgery for recurrent polypous sinusitis at the initial visit. In the postoperative period, all patients received treatment according to one of two schemes. The first scheme included antihistamines and topical steroids, the second additionally included clarithromycin. The patients underwent bacteriological examination of smears for flora and fungi from the cavity; cytological examination of smears-prints from the mucous membrane of the nasal cavity (we counted neutrophils, eosinophils and lymphocytes in the preparation); immunological and allergic examination (skin tests, serum specific IgE levels).</p></sec><sec><title>Results and discussion</title><p>Results and discussion. It has been established that the presence of a carriage and sensitization to S. aureus in a patient, especially in combination with atopy, results in a more severe course of polyposis sinusitis severe nasal obstruction (91% versus 69.5%, respectively) and is characterized by more pronounced eosinophilic inflammation than Polypoid Sinusitis associated with infection with other microorganisms (the number of eosinophils in the blood: 7.5% and 4.3%, respectively; the level of eosinophils in smears from the nasal mucosa is 13 and 4.8%, respectively; the average ECP level is 17.6 and 8.1 ug / ml, respectively).</p></sec><sec><title>Conclusions</title><p>Conclusions. The use of topical GCS remains one of the leading methods for preventing recurrence of PS.
