Клиническое течение и исход у пациентов с коморбидной патологией имеют ряд особенностей, требующих изучения. Актуальным в данном направлении является изучение биомаркеров, изменение уровней которых отражает выраженность патогенетических изменений и имеет прогностическое значение. Сердечно-сосудистые заболевания (ССЗ) и хроническая обструктивная болезнь легких (ХОБЛ) – одна из наиболее часто регистрируемых коморбидных патологий. Это обусловлено общими взаимоотягощающими звеньями патогенеза и факторами риска.Цель. Изучить и проанализировать в сравнительном аспекте уровень белка Клото у пациентов с инфарктом миокарда (ИМ) и с ИМ на фоне ХОБЛ в зависимости от распространенности поражения коронарных артерий (КА) по данным коронарографии.Материалы и методы. Обследовано 110 пациентов, разделенных на две группы: основная группа – пациенты с ИМ на фоне ХОБЛ (n = 60) и группа сравнения – пациенты с ИМ (n = 50). В качестве группы контроля были обследованы соматически здоровые лица (n = 30). Определение содержания уровня белка Клото в образцах плазмы осуществлялось методом иммуноферментного анализа.Результаты. Установлено, что у пациентов с ИМ на фоне ХОБЛ уровень белка Клото статистически значимо ниже, чем в группе контроля и у пациентов с ИМ. Выявлена взаимосвязь уровня белка Клото с показателями липидного профиля как у пациентов с ИМ, так и у пациентов с ИМ на фоне ХОБЛ с большей силой взаимосвязей у пациентов с коморбидной патологией. Установлено, что в группе пациентов с коморбидной патологией у лиц с однососудистым, двухсосудистым, трехсосудистым и более поражением КА уровень Клото был статистически значимо ниже, чем у пациентов с ИМ с соответствующим количеством пораженных КА.Заключение. Полученные результаты свидетельствуют о перспективности и необходимости дальнейшего изучения патогенетической роли уровня белка Клото у пациентов с кардиореспираторной коморбидностью.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. The clinical course and outcome in patients with comorbid pathology has a number of features that require further investigation. The investigation of biomarkers is important in this area as any change in their levels reflects the severity of pathogenetic changes and has prognostic value. Cardiovascular disease (CVD) and chronic obstructive pulmonary disease (COPD) are one of the most frequently reported comorbid pathologies. This is due to a large number of common interrelated links of pathogenesis and risk factors.The aim of the study. To study and analyze, in a comparative aspect, the level of Klotho protein in patients with MI and patients with MI and COPD, in relation to the number of stenosis of coronary arteries (CA) according to coronary angiography.Materials and methods. 110 patients were examined, divided into two groups: the main group - patients with MI on the background of COPD (n = 60) and the comparison group – patients with MI (n = 50). Somatically healthy individuals (n = 30) were examined as a control group. Determination of Klotho protein level in plasma samples was carried out by enzyme-linked immunosorbent assayResults. It was found that in patients with MI and COPD, the level of Klotho protein was statistically significantly lower than in the control group and in patients with MI. The relationship between the Klotho protein level and lipid profile parameters was revealed in both patients with MI and patients with MI and COPD, with a greater strength of relationships in patients with comorbid pathology. It was also found that in the group of patients with comorbid pathology in individuals with lesions of one, two, three or more CA, the Klotho level was statistically significantly lower than in patients with MI with the corresponding number of affected CA.Conclusions.
