Хронический гастрит – одна из наиболее частых проблем в гастроэнтерологии и общей врачебной практике. В данном исследовании мы попытались проанализировать действия врачей-терапевтов в  отношении пациентов с  симптомами диспепсии, оценить, как различный симптомокомплекс влияет на выбор терапии и формулировку диагноза.</p></sec><sec><title>Цель</title><p>Цель. Выявить особенности постановки предварительного диагноза по симптомам, выяснить возможности врачей дифференцировать клинические синдромы, а также определить подходы к эмпирической терапии (на первом приеме) и следование стратегии «тестируй и лечи», направленной на выявление инфекции, вызванной H. pylori.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование было проведено методом CAWI (Computer-assisted Web Interviewing): каждый врач, давший согласие на участие в исследовании, получил ссылку для прохождения опроса и самостоятельно внес ответы на сформулированные исследователем типовые вопросы. В рамках опроса врачам было предложено описание трех профилей взрослых пациентов с подозрением на диагноз «гастрит», впервые обратившихся с жалобами. Пациенты отличались доминирующими синдромами. Пациент № 1 с болевым синдром и симптомами диспепсии, пациент № 2 – с симптомами диспепсии и пациент № 3 – с болевым синдром.</p></sec><sec><title>Результаты</title><p>Результаты. В исследовании приняли участие 205 терапевтов из 33 городов России. Наиболее часто пациентам выставлялся предварительный диагноз «гастрит»: 50% врачей предположили гастрит для пациента № 1, 51% – для пациента № 2, 40% – для пациента № 3. Несмотря на однозначное описание симптомов замедленной моторики у пациентов № 1 и № 2, предполагаемые диагнозы чаще не отражают наличие этой проблемы. В то же время у пациентов № 1 и № 3 с вероятной симптоматикой гастрита в половине случаев врачи не предлагали проведение диагностики инфекции H. pylori. В случае выявления инфекции врачи предлагали назначение препаратов для эрадикации каждому пациенту, при этом наиболее популярной фармакологической группой были ИПП (91%). Однако каждый десятый врач предполагал проведение эрадикации без ИПП.</p></sec><sec><title>Выводы</title><p>Выводы. Результаты опроса свидетельствуют о недостаточной осведомленности врачей-терапевтов о симптомах замедленной моторики. Это приводит к назначению симптоматической фармакотерапии, не обеспечивающей разрыв патогенетических механизмов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Chronic gastritis is one of the most common problems in gastroenterology and general medical practice. In this study, we tried to analyze the actions of therapists in relation to patients with symptoms of dyspepsia, to assess how different symptom complex affects the choice of therapy and the diagnostics.</p></sec><sec><title>Aim</title><p>Aim. Identifying the process of a preliminary diagnosis by symptoms, determining the ability of physicians to differentiate clinical syndromes, and determining the approaches to empirical therapy at first visit and following the “test and treat” strategy aimed at identifying the H. pylori infection.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was conducted using the CAWI (Computer-assisted Web Interview) method. Each physician who agreed to participate in the study received a link to take the survey and independently entered the answers to the questions formulated by the researcher. The descriptions of three profiles of adult patients with suspected gastritis diagnosis who applied for the first time were suggested: 1) a patient with a pain syndrome and dyspepsia symptoms; 2) a patient with dyspepsia symptoms; 3) a patient with a pain syndrome.</p></sec><sec><title>Results</title><p>Results. The study involved 205 physicians from 33 cities of Russia. The most commonly assumed diagnosis was gastritis: 50% of responders suggested it for patient 1, 51% – for patient 2, and 40% – for patient 3. Despite the unambiguous description of the delayed motility symptoms in patients 1 and 2, the most commonly assumed diagnoses did not reflect the presence of a problem. At the same time, physicians did not conduct diagnostic tests for H. pylori infection in case of patients 1 and 3 with obvious symptoms of gastritis. In case of infection, physicians suggest prescribing eradication drugs to each patient, and the most popular pharmacological group suggested were PPIs (91%). However, every tenth physician suggested eradication without PPIs.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of the survey indicate a lack of awareness among therapists about the symptoms of delayed motility.
