Хронический риносинусит – распространенное заболевание, встречающееся повсеместно, характеризующееся воспалением слизистой оболочки околоносовых пазух и полости носа. При невозможности устранения причин развития заболевания консервативным путем и при рецидивирующем течении показано применение хирургического лечения для восстановления нормальной работы естественных соустий, в т. ч. остиомеатального комплекса для улучшения вентиляции и дренажа околоносовых пазух, и носового дыхания для купирования хронического воспаления.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить степень интраоперационного кровотечения у пациентов с хроническим риносинуситом.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. На базе ФГБУ СПБ НИИ ЛОР МЗ РФ в период с 2020 по 2021 г. проведено исследование влияния местных интраназальных глюкокортикостероидов в интраоперационном периоде у пациентов с хроническим полипозным риносинуситом. Анализировалось суммарное количество кровопотери за операцию по поводу хронического полипозного риносинусита у пациентов, использовавших мометазона фуроат предоперационно, а также не принимающих топических гормональных препаратов.</p></sec><sec><title>Результаты</title><p>Результаты. В группе исследования средняя кровопотеря за операцию составила 257 мл, в контрольной – 401 мл. У пациентов, которые принимали интраназальные глюкокортикостероиды, во время операции интенсивность кровотечения менее выражена. Несомненно, существует множество причин, влияющих на интенсивность кровотечения в целом, однако снижение воспалительной реакции в полости носа и снижение метаболитов воспаления благоприятно влияет на состояние слизистой оболочки полости носа.</p></sec><sec><title>Заключение</title><p>Заключение. Применение интраназальных глюкокортикостероидов в предоперационном периоде позитивно сказывается на снижении воспалительного процесса в полости носа, что способствует снижению интенсивности интраоперационного кровотечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Chronic rhinosinusitis is a disease that occurs everywhere, characterized by inflammation of the mucous membrane of the paranasal sinuses and nasal cavity. If it is impossible to eliminate the causes of the recurrent disease with medical therapy, the use of surgical treatment help to recover the normal functioning of the ostium, including the ostiomeatal complex to improve ventilation and drainage of the paranasal sinuses and recover nasal breathing to relieve chronic inflammation.</p></sec><sec><title>Purpose of the study</title><p>Purpose of the study. Assessment of the degree of intraoperative bleeding in patients with chronic rhinosinusitis.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. On the basis of the FGBU SPB Research Institute of ENT of the Ministry of Health of the Russian Federation in the period from 2020 to 2021. the study of the effect of local intranasal glucocorticosteroids (INGKS) in the intraoperative period in patients with chronic polypous rhinosinusitis was carried out. The total amount of blood loss during surgery for chronic polypous rhinosinusitis in patients who used mometasone furoate preoperatively and in patients not taking topical hormonal drugs was analyzed.</p></sec><sec><title>Results</title><p>Results. In the study group, the average blood loss per operation was 257 ml, in the control group – 401 ml. In patients who took intranasal glucocorticosteroids during surgery, the intensity of bleeding is less pronounced. Undoubtedly, there are many reasons that affect the intensity of bleeding in general, however, a decrease in the inflammatory response in the nasal cavity and a decrease in inflammatory metabolites have a beneficial effect on the state of the nasal mucosa.</p></sec><sec><title>Conclusion</title><p>Conclusion.
