Распространенной клинической формой ДПН является дистальная симметричная, преимущественно сенсорная, полиневропатия. Основу патогенеза ДПН составляет хроническая гипергликемия, которая приводит к накоплению продуктов избыточного гликирования в различных органах и тканях, в т. ч. к поражению vasa nervorum, участвующим в кровоснабжении периферических нервов. Тщательный подход к диагностике ДПН обеспечивает своевременное выявление данной патологии и начало этиотропной и патогенетической терапии, что достоверно снижает скорость прогрессирования ДПН, а на ранних стадиях приводит к регрессу клинических проявлений заболевания. Длительная терапия сахароснижающим препаратом – Метформином – нередко приводит к развитию дефицита витаминов группы B вследствие нарушениявсасывания в кишечнике.В работе представлено наблюдение пациентки, 68 лет, с СД 2-го типа с клиническими проявлениями диабетической и дефицитарной невропатии. Наряду с медикаментозной коррекцией СД проводилась патогенетическая терапия ДПН препаратом альфа-липоевой кислоты и комбинированным препаратом, содержащим витамины B1, B6, B12. Также приведено обсуждение эффективности комплексной патогенетически обоснованной терапии, выраженной ослаблением клинических проявлений ДПН и регрессом сенситивной атаксии.</p></abstract><trans-abstract xml:lang="en"><p>Diabetic polyneuropathy (DPN) is the most common neurological complication of diabetes mellitus. The most common type of DPN is distal symmetric, predominantly sensory, polyneuropathy. Chronic hyperglycemia plays a key role in DPN pathogenesis. It leads to accumulation of advanced glycation end products in tissues, including vasa nevrorum that supply peripheral nerves with oxygen. Thorough diagnostic approach to DPN is crucial for early detection of this condition. Early diagnosis and treatment slow DPN progression rates and leads to symptom reduction. Long-lasting metformin therapy may cause vitamin B deficiency. This article describes a comprehensive approach to the treatment of patient with type 2 diabetes and symptomatic diabetic and vitamin deficiency neuropathy. Apart from DM therapy, the patient received pathogenesis-based therapy of DPN with alpha-lipoic acid and a combination of vitamins B1, B6 and B12.
