Псориатическая болезнь – это гетерогенное воспалительное заболевание с различными клиническими проявлениями, включающими бляшечный псориаз и псориатический артрит. Было показано, что повышенный уровень TNF-α наблюдается как при псориатическом артрите, так и при псориазе. Изучение закономерности экспрессии гена TNF-α может помочь в дифференциальной диагностике псориатического артрита и псориаза.</p><p>Цель – изучить закономерность экспрессии гена TNF-α в мононуклеарных клетках крови больных псориатическим артритом и псориазом для возможной дифференциальной диагностики двух патологий.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Выделение мононуклеарных клеток проводили из периферической крови 31 пациента с псориазом бляшечного типа, 45 пациентов с псориатическим артритом и 20 здоровых людей из контрольной группы. Экспрессию гена TNF-α анализировали методом ПЦР в реальном времени.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. В результате сравнения было выявлено, что уровень экспрессии TNF-α у больных псориатическим артритом в 179 раз превышает уровень экспрессии у здоровых волонтеров. Уровень экспрессии TNF-α у больных псориазом также значительно  (в  106  раз) превышал уровень экспрессии у  здоровых людей. Удалось выявить достоверную разницу между пациентами с псориатическим артритом и псориазом.</p></sec><sec><title>Выводы</title><p>Выводы. Больные псориазом по уровню экспрессии гена TNF-α в мононуклеарных клетках приближены к состоянию пациентов с псориатическим артритом. Высокий уровень экспрессии гена TNF-α может стать маркером возможного поражения суставов у больных псориазом и сигналом для пересмотра терапевтического подхода к конкретному пациенту. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Psoriatic disease is a heterogeneous inflammatory disease with different clinical manifestations, including plaque psoriasis and psoriatic arthritis. It has been shown that elevated levels of TNF-α are observed in both psoriatic arthritis and psoriasis. Studying the TNF-α gene expression pattern can help in the differential diagnosis between psoriatic arthritis and psoriasis.</p><p>The objective is to study the TNF-α gene expression pattern in blood mononuclear cells of patients with psoriatic arthritis and psoriasis for possible differential diagnosis between these two diseases.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Mononuclear cells were isolated from the peripheral blood of 31 patients with plaque psoriasis, 45 patients with psoriatic arthritis and 20 healthy controls. The expression level of the TNF-α gene was analysed using a real-time PCR method.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. As a result of the comparison, the expression level of TNF-α in patients with psoriatic arthritis was found to be 179 times higher than the expression level in healthy volunteers. The expression level of TNF-α in patients with psoriasis was also significantly (106 times) higher than the expression level in healthy people. We managed to identify a significant difference between patients with psoriatic arthritis and psoriasis.</p></sec><sec><title>Conclusions</title><p>Conclusions. Patients with psoriasis in terms of TNF-α gene expression level in mononuclear cells are close to the condition of patients with psoriatic arthritis.
