В  течение почти 60  лет антагонисты витамина К  (АВК) были основой антикоагулянтной терапии, но  в  последние годы антикоагулянтами выбора стали прямые пероральные антикоагулянты (ПОАК), т. к. они имеют множество хорошо известных преимуществ: более предсказуемый антикоагулянтный эффект, отсутствие необходимости в  подборе дозы  (есть необходимость в  коррекции дозы только при нарушениях функции почек), рутинного лабораторного контроля фармакодинамического эффекта (за исключением особых клинических ситуаций), меньшая частота клинически значимых взаимодействий с лекарственными средствами по  сравнению с  варфарином и  меньшая зависимость от  генетических особенностей пациента. Основными показаниями к ПОАК являются: профилактика венозной тромбоэмболии у пациентов, перенесших эндопротезирование суставов нижних конечностей, профилактика инсульта и системной эмболии у пациентов с фибрилляцией предсердий, лечение и профилактика рецидивирующего тромбоза глубоких вен (ТГВ) и тромбоэмболии легочной артерии (ТЭЛА). Введение прямых пероральных антикоагулянтов долгое время считалось крупным терапевтическим достижением в  основном потому, что они не требуют терапевтического мониторинга. Несмотря на это, ПОАК, как и антагонисты витамина К, все еще могут вызывать большие и  клинически значимые небольшие кровотечения даже при правильном применении. Учитывая, что пациенты с  ПОАК часто старше и  имеют множественные сопутствующие заболевания, широко распространена полипрагмазия. Лекарственные взаимодействия с участием ПОАК вносят важный вклад в повышенный риск кровотечения. Осведомленность об этих лекарственных взаимодействиях и способах их решения имеет решающее значение для оптимизации лечения при одновременном снижении риска кровотечения. В этом обзоре представлен обзор метаболизма ПОАК, наиболее распространенных препаратов, которые могут взаимодействовать с ПОАК, и способов устранения этих взаимодействий. </p></abstract><trans-abstract xml:lang="en"><p>Cardiovascular disease is the most common cause of death in the world. For almost 60 years vitamin K antagonists (VKAs) have been the mainstay of anticoagulant therapy, but in recent years direct oral anticoagulants (DAACs) have become the anticoagulant of choice, as they have many well-known advantages: more predictable anticoagulant effect, no need for dose selection (there is  a  need for  dose adjustment only for  renal dysfunction), routine laboratory monitoring of  pharmacodynamic effect  (except in special clinical situations), less frequency of clinically significant drug interactions compared with warfarin, and less dependence on patient genetic characteristics. The main indications for POAC are: prevention of venous thromboembolism in patients who have undergone endoprosthesis of lower limbs, prevention of stroke and systemic embolism in patients with atrial fibrillation, treatment and prevention of recurrent deep vein thrombosis (DVT) and pulmonary embolism. The administration of direct oral anticoagulants (DOACs) has long been considered a major therapeutic advance, mainly because they do not require therapeutic monitoring. Despite this, POACs, like vitamin K antagonists, can still cause major and clinically significant minor bleeding, even when used correctly. Considering that POAC patients are often older and have multiple comorbidities, polypragmasy is widespread. Drug interactions involving POACs are important contributors to the increased risk of bleeding. Awareness of these drug interactions and how to address them is critical to optimizing treatment while reducing the risk of bleeding.
