Научные дискуссии о способах повышения эффективности и безопасности терапии остеоартрита не прекращаются. В статье рассматриваются наиболее дискутабельные вопросы и представлены результаты исследования применения гиалуроната натрия отечественного производства при гонартрозе.Цель. Сравнить клиническую эффективность, переносимость и безопасность применения протеза синовиальной жидкости отечественного производства и препаратов группы SYSADOA у больных гонартрозом. Дизайн исследования. Проспективное, сравнительное, рандомизированное, одноцентровое. Длительность исследования составила 12 мес.Материалы и методы. В исследование включено 180 больных гонартрозом, рандомизированных методом таблицы случайных чисел на три группы. Группа исследования: 50 пациентов, получавших раствор гиалуроната натрия Рипарт® внутрисуставно. Группа сравнения: 30 пациентов, получавших SYSADOA внутрисуставно. Группа контроля: 100 пациентов, получавших SYSADOA внутримышечно.Результаты. Наиболее быстрое и выраженное снижение индекса ВАШ получено на фоне терапии гиалуронатом натрия: на 1-й нед. на 31%, к концу 2-й нед. на 48%, с максимумом снижения к 3-му мес. на 49,1% (p &lt; 0,001). Через 12 мес. наблюдался меньший уровень индекса ВАШ: на 32% ниже, чем до начала терапии (t = 47,23; p &lt; 0,001). Сопоставимые результаты наблюдались в этой группе по динамике индекса AUSCAN/WOMAC. Эффективность терапии в группах сравнения и контроля была ниже (p &lt; 0,05). Кроме того, применение гиалуроната натрия обеспечивало достоверное снижение потребности в приеме НПВП: 20 пациентов полностью прекратили прием НПВП (40%), 25 (50%) стали реже принимать НПВП или уменьшили дозу препаратов в два раза, через 12 мес. продолжали прием НПВП лишь 20% больных этой группы (p &lt; 0,05).Выводы. Таким образом, Рипарт® продемонстрировал высокую эффективность в снижении болевого синдрома, способствовал достоверному уменьшению потребности в приеме НПВП у больных гонартрозом и обладает хорошим профилем безопасности.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Scientific discussions about ways to improve the effectiveness and safety of osteoarthritis (OA) therapy do not stop. The article discusses the most controversial issues of the problem and presents the results of a study on the use of sodium hyaluronate in gonarthrosis.Aim. To compare the clinical efficacy, tolerability and safety of the use of synovial fluid substitute of domestic manufacture and symptomatic slow-acting drugs (SYSADOA) in patients with gonarthrosis. Study design. Prospective, comparative, randomized, single center study. The duration of the study was 12 months.Materials and methods. The study included 180 patients with gonarthrosis, randomized into three groups. Study group: 50 patients treated with Ripart® sodium hyaluronate solution intra-articularly. Comparison group: 30 patients treated with SYSADOA, intraarticular. Control group: 100 patients treated with SYSADOA intramuscularly.Results. The most rapid and pronounced decrease in the VAS index was obtained during sodium hyaluronate therapy: by 31% at the 1st week, by 48% by the end of the 2nd week, with a maximum decrease by 49.1% by the 3rd month (p &lt; 0.001). After 12 months, a lower level of the VAS index was observed: 32% lower than before the start of therapy (t = 47.23; p &lt; 0.001). Comparable results were observed in this group in terms of the dynamics of the AUSCAN/WOMAC index. The effectiveness of therapy in the comparison and control groups was lower (p &lt; 0.05). In addition, the use of sodium hyaluronate provided a significant reduction in the need for taking NSAIDs: 20 patients completely stopped taking NSAIDs (40%), 25 (50%) – they began to take NSAIDs less often or reduced the dose of drugs by 2 times, after 12 months they continued taking NSAIDs only 20% of patients in this group (p &lt; 0.05).Сonclusions.
