Поддерживающая терапия дурвалумабом после окончания химиолучевой терапии при нерезектабельном немелкоклеточном раке легкого III стадии (НМРЛ) является новым стандартом лечения.</p></sec><sec><title>Цель</title><p>Цель. Изучить особенности и результаты применения поддерживающей терапии дурвалумабом после химиолучевой терапии у пациентов с нерезектабельным НМРЛ III стадии в реальной клинической практике в России.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В данное наблюдательное ретроспективное исследование было включено 50 пациентов с нерезекта-бельным НМРЛ III стадии после одновременной или последовательной химиолучевой терапии (ХЛТ). Медиана времени наблюдения при первичном анализе составила 12,4 мес.</p></sec><sec><title>Результаты</title><p>Результаты. Средний возраст пациентов в исследовании составил 61,2 года (58,4-64,1; 95% ДИ). Большинству пациентов проводилась последовательная ХЛТ (76%, n = 38). Медиана времени от окончания ХЛТ до начала приема дурвалумаба варьировала от 22 до 50 дней (в целом 35 дней). Расчетные медианы выживаемости без прогрессирования (ВБП) и общей выживаемости составили 10,86 мес. (7,78-14,01, 95% ДИ) и 26 мес. (20,19-31,81, 95% ДИ), соответственно. Отмечалась тенденция к увеличению ВБП у пациентов с курением в анамнезе: 12 мес. (9,79-14,2; 95% ДИ) против 4,9 мес. (0,0-12,47; 95% ДИ), р = 0,2. При этом у половины пациентов без курения в анамнезе (5/10) были выявлены активирующие мутации (EGFR в 19-м экзоне, ALK, ROS1, cMET). Наиболее частыми нежелательными явлениями, представляющими особый интерес, были пневмониты 1-2-й ст. (36%, n = 18), что привело к окончательному прекращению лечения у 6% пациентов (n = 3). Не сообщалось о случаях нежелательных явлений 3-4-й ст.</p></sec><sec><title>Выводы</title><p>Выводы. Реальные характеристики пациентов в нашем исследовании отличались от данных исследования PACIFIC. Последовательная ХЛТ является наиболее частым методом лечения местнораспространенного нерезектабельного НМРЛ в России. Расчетная ВБП была короче, чем в PACIFIC, но при этом было зафиксировано и меньше случаев пневмонитов, в т. ч. приведших к прекращению лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The addition of durvalumab after chemoradiation therapy in unresectable stage III non-small-cell lung cancer (NSLC) is a new standard of care.</p></sec><sec><title>Aim</title><p>Aim. Study the features and outcomes of durvalumab maintenance treatment after chemoradiotherapy in patients with unresectable stage III NSCLC in the real-world clinical practice in Russia.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 50 patients with unresectable III stage NSCLC after concurrent or sequential chemoradiotherapy (CRT) were enrolled in this observational retrospective study. Median follow up time at primary analysis was 12.4 months.</p></sec><sec><title>Results</title><p>Results. A mean age of the patients in the study was 61.2 years (58.4-64.1; 95% CI). Most of the patients had received sequential CRT (76%, n = 38). Median time of durvalumab start from the end of CRT varied from 22 to 50 days (overall - 35 days). Estimated median PFS and OS were 10.86 months (7.78-14.01, 95% CI) and 26 months (20.19-31.81, 95% CI), respectively. There was a trend toward increased PFS in patients with smoking history: 12 months (9.79-14.2; 95% CI) versus 4,9 months (0.0-12.47; 95% CI), p = 0.2. Half of the patients without smoking history (5/10) had targetable mutations (EGFR ex 19, ALK, ROS1, cMET). Most common reported adverse events of special interest were pneumonitis grade 1-2 (36%, n = 18), leading permanent treatment discontinuation to in 6% of patients (n = 3). There were no reported cases of grade 3-4 adverse events.</p></sec><sec><title>Conclusions</title><p>Conclusions. Real-world characteristic of patients in our study were different from PACIFIC trial. Sequential CRT is the most frequent treatment option in locally advanced unresectable NSCLC in Russia.
