В  работе представлены результаты исследования эффективности применения препарата этилметилгидроксипиридина малат у пациентов с long COVID с основой на динамике клинических проявлений и метаболомных показателей, ответственных за уровень оксидативного стресса. Цель. Оценить эффективность и выявить нежелательные реакции при применении препарата этилметилгидроксипиридина малат для снижения уровня окислительного стресса и уменьшения симптомов астении и когнитивных нарушений у пациентов с long COVID. Материалы и методы. В нерандомизированное контролируемое проспективное исследование включены 24 пациента с диагнозом U09.9 «Состояние после COVID-19 неуточненное» по МКБ-10. Пациенты были разделены на две группы по  12  чел. Экспериментальная группа получала этилметилгидроксипиридина малат таблетки жевательные 100  мг в дозировке 400 мг/сут. Клиническая оценка по шкалам (шкала одышки mMRC, тест 6-минутной ходьбы (6MWT), субъективная шкала оценки астении MFI-20, Монреальская шкала оценки когнитивных функций (MoCA), шкала оценки базовой функциональной активности Бартел) и  определение метаболомных показателей проводились на  1-й  и  14–18-й день исследования. Результаты. Применение препарата этилметилгидроксипиридина малат у пациентов с long COVID привело к снижению уровня оксидативного стресса и нормализации функции митохондрий, а также более выраженным улучшениям клинической картины. По шкале оценки астении (MFI-20) в группе пациентов, получавших этилметилгидроксипиридина малат, отмечается улучшение на 21%, в группе контроля – на 13%. По шкале оценки когнитивных нарушений (MoCA) в группе препарата показатели улучшаются на 20%, в группе контроля – на 12,5%. Заключение. Препарат этилметилгидроксипиридина малат таблетки жевательные 100 мг в дозировке 400 мг/сут демонстрирует клиническую эффективность, характеризующуюся в  том числе снижением уровня оксидативного стресса, а также клиническую безопасность ввиду отсутствия развития нежелательных реакций у пациентов с long COVID.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. The material presents the results of the study of the effectiveness of the drug Ethylmethylhydroxypyridine malate in  patients with long COVID, based on the  dynamics of  clinical manifestations and metabolomic parameters responsible for the level of oxidative stress. Aim. Evaluation of efficacy and registration of adverse reactions of ethylmethylhydroxypyridine malate administration to decrease oxidative stress level and reduce symptoms of asthenia and cognitive disorders in patients with long COVID. Materials and methods. 24 patients diagnosed with U09.9 “Condition after COVID-19 unspecified” were included in a non-randomized controlled prospective study, Patients were divided into 2  groups of  12  each. The experimental group received Ethylmethylhydroxypyridine malate tablets chewable 100 mg in a dosage of 400 mg/day. Clinical assessment by scales (mMRC dyspnea scale; 6-minute walk test  (6MWT); MFI-20  subjective asthenia assessment scale; Montreal Cognitive Assessment Scale (MoCA); Barthel Baseline Functional Activity Rating Scale), and determination of metabolomic parameters were performed on days 1 and 14–18 of the study. Results. Use of the drug ethylmethylhydroxypyridine malate in patients with long COVID, led to a decrease in the level of oxidative stress, and normalization of  mitochondrial function, as well as more pronounced improvements of  the clinical picture. According to the scale of evaluation of asthenia (MFI-20) in the group of patients who received ethylmethylhydroxypyridine malate improvement of 21% was noted, in the control group improvement of 13% was noted. According to cognitive impairment assessment scale (MoCA), the drug group showed 20% improvement, while the control group showed 12,5% improvement. Conclusion.
