Декомпенсированный цирроз печени (ЦП) зачастую сопровождается явлениями печеночной энцефалопатии (ПЭ). Роль нарушения структурно-функциональных свойств альбумина в развитии данной патологии в настоящее время не определена.</p></sec><sec><title>Цель</title><p>Цель. Оценить влияние заместительной терапии альбумином на клинические проявления ПЭ и выраженность структурных и функциональных изменений сывороточного альбумина.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включили 28 совершеннолетних пациентов с декомпенсированным ЦП и ПЭ разделенных на две сопоставимые группы, получающие базисную терапию ЦП (стандартная медикаментозная терапия) и дополнительно – заместительную терапию альбумином (стандартная медикаментозная терапия + альбумин). Помимо стандартного обследования, проводилась оценка свойств молекулы альбумина методом ЭПР-теста. Динамика показателей оценивалась в группах стандартного лечения и стандартного лечения + альбумин. Математическая обработка проводилась в программе StatTech v. 3.1.4 (разработчик ООО «Статтех», Россия).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Выявлено, что у больных с декомпенсированным циррозом печени наблюдаются серьезные нарушения конфигурации молекулы альбумина, которые ведут к снижению его функциональной активности. Способность к связыванию, транспорту и детоксикации была нарушена у подавляющего большинства больных. Нативная конформация молекулы была патологической у всех пациентов. На фоне заместительной терапии альбумином указанные нарушения способны восстанавливаться до нормальных значений. Вместе с тем мы обнаружили, что заместительная терапия альбумином способствует снижению выраженности печеночной энцефалопатии в большей степени, чем стандартное лечение.</p></sec><sec><title>Выводы</title><p>Выводы. Нарушение структурно-функциональных свойств молекулы альбумина связано с прогрессированием печеночной энцефалопатии у пациентов с циррозом печени. Трансфузии высококонцентрированного альбумина человека приводят к нормализации свойств молекулы и регрессии явлений энцефалопатии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. It is often observed that decompensated liver cirrhosis is accompanied by hepatic encephalopathy. The role of violation of the structural and functional properties of albumin in the development of this pathology is currently not defined.</p></sec><sec><title>Aim</title><p>Aim. Was to evaluate the effect of albumin replacement therapy on the clinical manifestations of hepatic encephalopathy and the severity of structural and functional changes in serum albumin.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 28 adult patients with decompensated cirrhosis and hepatic encephalopathy divided into two comparable groups. In addition to the standard examination, the properties of the albumin molecule were evaluated using the EPR test. The dynamics of indicators was assessed in the groups of standard treatment and standard treatment + albumin. Mathematical processing was carried out in the StatTech v. 3.1.4 (developer Stattech LLC, Russia).</p></sec><sec><title>Results and discussion</title><p>Results and discussion. It was revealed that patients with decompensated liver cirrhosis have serious disturbances in the albumin configuration, which lead to a decrease in its functional activity. The ability to bind, transport and detoxify were impaired in the vast majority of patients. The native conformation of the molecule was abnormal in all patients. Against the background of albumin replacement therapy, these disorders are able to recover to normal values. Along with this, we found that albumin replacement therapy reduced the severity of hepatic encephalopathy to a greater extent than standard treatment.</p></sec><sec><title>Conclusion</title><p>Conclusion. Violation of the structural and functional albumin properties is associated with the progression of hepatic encephalopathy in patients with liver cirrhosis.
