Наиболее значимым этиологическим фактором гастрита на сегодняшний день можно считать инфекцию H. pylori. Частым клиническим проявлением может быть синдром диспепсии, который обусловлен нарушениями моторики. Симптоматика может существенно влиять на качество жизни пациента, что требует быстрой и эффективной фармакотерапии. В данной работе обсуждается алгоритм действий практикующего врача в случае пациента с необследованной диспепсией. Следует помнить о негативном влиянии ИПП на точность результатов диагностических тестов на H. pylori. В связи с этим предлагается на этапе диагностического поиска использовать эмпирическую терапию прокинетиками. Среди доступных на российском рынке прокинетиков выгодно отличается итоприда гидрохлорид благодаря высокому профилю безопасности и доказанной эффективности. Современная доказательная база показывает возможность использования прокинетика Ганатон® (итоприда гидрохлорид) в качестве эмпирической терапии диспепсии неуточненной этиологии, в т. ч. и пациентам с предварительным диагнозом «гастрит». Благодаря двойному механизму действия итоприда гидрохлорид облегчает симптомы диспепсии за счет улучшения эвакуации пищи из желудка и может быть применен на длительный период. Ряд исследований показали преимущество итоприда при лечении функциональной диспепсии по сравнению с другими прокинетиками, в т. ч. метоклопрамидом и домперидоном. Таким образом, назначение прокинетика Ганатон® (итоприда гидрохлорид) в качестве эмпирической терапии диспепсии неуточненной этиологии, включая пациентов с предварительным диагнозом «гастрит», является патогенетически обоснованным решением, направленным на улучшение состояния пациента в краткие сроки до установки окончательного клинического диагноза.</p></abstract><trans-abstract xml:lang="en"><p>Chronic gastritis is a complex, polyetiological pathology with no clear clinical presentation. The most significant etiological factor of gastritis to date is H. pylori infection. A common clinical manifestation is the dyspepsia syndrome, which is caused by impaired motility. Symptoms can significantly affect a patient’s quality of life, necessitating rapid and effective pharmacotherapy. This paper discusses the algorithm of the physician actions in the case of a patient with uninvestigated dyspepsia. PPI has significant negative impact on the accuracy of H. pylori diagnostic test results. In this regard, it is proposed to use empirical therapy with prokinetics before diagnostic test would be performed. Among the prokinetics available on the Russian market, itopride hydrochloride stands out due to its high safety profile and proven efficacy. Current evidence supports the use of the prokinetic Ganaton® (itopride hydrochloride) as empirical therapy for dyspepsia of undetermined etiology, including patients with a preliminary diagnosis of gastritis. Due to its dual mechanism of action, itopride hydrochloride alleviates dyspeptic symptoms by improving gastric evacuation and can be used for an extended period. Several studies have shown the superiority of itopride in treating functional dyspepsia compared to other prokinetics, including metoclopramide and domperidone.
