Изучение спектра нейрокогнитивных нарушений у пациентов с артериальной гипертензией (АГ) с целью создания эффективной лечебной и реабилитационной стратегии является актуальным направлением современной медицины.</p></sec><sec><title>Цель</title><p>Цель. Изучить нейропсихологические характеристики у пациентов с АГ в среднем и пожилом возрасте.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовано 357 пациентов среднего и пожилого возраста с АГ. Всем пациентам проведено нейропсихологическое обследование: Монреальская шкала оценки когнитивных функций (MoCA-тест), тест таблицы Шульте, тест вербальных ассоциаций, тест слежения (Trail Making Test – TMT), самоопросник астении (MFI-20), тест Ридера в адаптации О. Копиной, опросник на уровень жизненного истощения, госпитальная шкала тревоги и депрессии (HADS).</p></sec><sec><title>Результаты</title><p>Результаты. Анализ результатов общего нейропсихологического тестирования показал отклонение от референсных значений у большинства участников. В тесте Шульте отмечено увеличение времени выполнения у 50% пациентов пожилого и у 21% среднего возраста; в тесте TMT: увеличение времени выполнения части А – у 88% пациентов пожилого возраста и 58% пациентов среднего возраста, части В – у 97 и 88% пациентов соответственно. МОСа-тест продемонстрировал выраженные когнитивные нарушения у 16% пациентов среднего возраста и у 35% пациентов пожилого. Более 97% пациентов пожилого и 88% среднего возраста показали высокий уровень астении в тесте MFI-20; жизненное истощение отмечено у 56 и 45%, а тревога и депрессия – более чем у 50% пациентов пожилого и 35% среднего возраста соответственно.</p></sec><sec><title>Заключение</title><p>Заключение. В изучаемых группах пациентов с АГ отмечается снижение интегрального показателя когнитивных функций, а также изменения показателей в тестах, характеризующих состояние управляющих функций, внимания, скорости мыслительных процессов и семантической памяти, при этом более выраженные отклонения отмечались у лиц пожилого возраста. Описанные когнитивные нарушения сочетались с высоким уровнем психоэмоционального напряжения, тревожности, депрессии и астении.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The study of the spectrum of neurocognitive disorders in patients with arterial hypertension (AH) in order to create an effective therapeutic and rehabilitation strategy is an urgent direction of modern medicine.</p></sec><sec><title>Aim</title><p>Aim. To study neuropsychological characteristics in patients with hypertension in middle and old age.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 357 middle-aged and elderly patients with hypertension were examined. All patients underwent neuropsychological examination: Montreal Cognitive Function Assessment Scale (MoCA test), Schulte Table Test, Verbal Association Test, Trail Making Test (TMT), Asthenia Self-questionnaire (MFI-20), O. Kopina Reader Adaptation Test, questionnaire on the level of life exhaustion, hospital anxiety scale and depression (HADS).</p></sec><sec><title>Results</title><p>Results. The analysis of the results of general neuropsychological testing showed a deviation from the reference values in the majority of participants. In the Schulte test, an increase in execution time was noted in 50% of elderly patients and in 21% of middle-aged patients. In the TMT test: an increase in the execution time of part A – in 88% of elderly patients and 58% of middle-aged patients, part B – in 97 and 88% of patients, respectively. The MoCA test demonstrated pronounced cognitive impairment in 16% of middle-aged patients and in 35% of elderly patients. More than 97% of elderly and 88% of middle-aged patients showed a high level of asthenia in the MFI-20 test; life exhaustion was noted in 56 and 45%, and anxiety and depression in more than 50% of elderly and 35% of middle-aged patients, respectively.</p></sec><sec><title>Conclusion</title><p>Conclusion. In the studied groups of patients with hypertension, there was a decrease in the integral index of cognitive functions, as well as changes in indicators in tests characterizing the state of control functions, attention, speed of thought processes and semantic memory, while more pronounced deviations were noted in the elderly.
