Роль и механизм влияния витамина D на течение хронических заболеваний легких у детей изучены не до конца. В частности, недостаточно исследований о влиянии обеспеченности витамином D на продукцию интерферона-γ (IFN-γ) у детей с муковисцидозом и бронхиальной астмой.</p></sec><sec><title>Цель</title><p>Цель. Проанализировать динамику показателей IFN-γ у детей с муковисцидозом и бронхиальной астмой в зависимости от уровня 25(OH)D сыворотки крови до дотации препаратами холекальциферола и на ее фоне.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследованы 114 детей (от 3 мес. до 18 лет), из них с бронхиальной астмой - 51 (44,7%), с муковисцидозом - 34 (29,8%), контрольная группа - 29 (25,5%) детей. При наличии гиповитаминоза D назначался трехмесячный курс холекальциферола, дозы в соответствии с рекомендациями национального консенсуса «Муковисцидоз: определение, диагностические критерии, терапия» и национальной программы «Недостаточность витамина D у детей и подростков Российской Федерации: современные подходы к коррекции». Всем детям двукратно определяли показатели 25(OH)D и IFN-γ.</p></sec><sec><title>Результаты</title><p>Результаты. Медианы 25(OH)D у пациентов с муковисцидозом и бронхиальной астмой не достигали уровня здоровых детей ни до, ни после дотации препаратами холекальциферола в рекомендованных консенсусами дозах. Прием витамина D приводил к снижению IFN-γ у здоровых детей с 3,07 [2,29; 4,81] пг/мл до 2,18 [1,74; 3,45] пг/мл (р &lt; 0,05), у пациентов детского возраста с муковисцидозом и бронхиальной астмой данных изменений в общей популяции выявлено не было, однако после дотации холекальциферола уровень IFN-γ был значимо выше у пациентов с муковисцидозом и бронхиальной астмой - 2,86 [2,13; 3,86] пг/мл и 3,11 [0,89; 5,0] пг/мл соответственно, чем у здоровых детей - 2,18 [1,74; 3,45] пг/мл (р &lt; 0,05). У девочек с муковисцидозом и у здоровых девочек отмечено статистически значимое снижение IFN-γ после дотации холекальциферола, в группах мальчиков значимых изменений уровней IFN-γ не получено.</p></sec><sec><title>Заключение</title><p>Заключение. Модуляция уровня IFN-γ, по-видимому, является одним из механизмов иммунотропного воздействия витамина D на течение хронического воспаления инфекционной (муковисцидоз) и аллергической (бронхиальная астма) этиологии у детей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The role and mechanism of the effect of vitamin D on the course of chronic lung diseases in children are not yet fully understood. In particular, there are not enough studies on the effect of vitamin D status on the production of interferon-γ (IFN-γ) in children with cystic fibrosis and bronchial asthma.</p></sec><sec><title>Aim</title><p>Aim. To analyse the changes in IFN-γ levels in children with cystic fibrosis and bronchial asthma according to the serum 25(OH)D level before and during cholecalciferol supplementation.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A total of 114 children (aged from 3 months to 18 years old) were examined, including 51 (44.7%) children with bronchial asthma, 34 (29.8%) children with cystic fibrosis, and 29 (25.5%) children of the control group. If hypovitaminosis D was diagnosed, the children received a three-month cholecalciferol course at doses in accordance with the recommendations of the national consensus - Cystic Fibrosis: Definition, Diagnostic Criteria, Therapy and the national program - Vitamin D Deficiency in Children and Adolescents of the Russian Federation: Current Approaches to Management. The 25(OH)D and IFN-γ levels were twice determined in all children.</p></sec><sec><title>Results</title><p>Results. Median 25(OH)D levels in patients with cystic fibrosis and bronchial asthma did not reach the level of healthy children either before or after supplementation with cholecalciferol drugs at the recommended doses. Vitamin D intake resulted in a decrease in IFN-γ levels in healthy children from 3.07 [2.29; 4.81] pg/ml to 2.18 [1.74; 3.45] pg/ml (p &lt; 0.05), whereas such changes were not detected in the paediatric patients with cystic fibrosis and bronchial asthma in the general population. However, after supplementation with cholecalciferol, the IFN-γ level was significantly higher in patients with cystic fibrosis and bronchial asthma - 2.86 [2.13; 3.86] pg/ml and 3.11 [0.89; 5.0] pg/ml, respectively, than in healthy children - 2.18 [1.74; 3.45] pg/ml (p &lt; 0.05). A statistically significant decrease in IFN-γ level was observed in girls with cystic fibrosis and in healthy girls after supplementation with cholecalciferol, whereas no significant changes in IFN-γ levels were detected in the groups of boys.</p></sec><sec><title>Conclusion</title><p>Conclusion.
