Хроническая обструктивная болезнь легких (ХОБЛ) – заболевание, характеризующееся персистирующим ограничением воздушного потока и являющееся следствием хронического воспаления дыхательных путей и ткани легкого в ответ на воздействие ингалируемых повреждающих частиц или газов. Высокая распространенность ХОБЛ и высокий риск летальности среди пациентов уже долгое время остаются актуальной проблемой современного здравоохранения. Одним из наиболее тяжелых осложнений ХОБЛ является хроническая дыхательная недостаточность (ХДН). Использование кислорода у пациентов с хронической гипокесмией должно быть постоянным, длительным и, как правило, проводиться в домашних условиях, поэтому такая форма терапии называется длительной кислородотерапией (ДКТ).</p></sec><sec><title>Цель</title><p>Цель. Изучить влияние применения ДКТ на выживаемость пациентов с ХОБЛ, осложненной ХДН.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 324 пациента в период с 2011 по 2017 г. с подтвержденным диагнозом ХОБЛ (средний ОФВ1 – 31,6% должных), применявших ДКТ. Клинические данные получены из историй болезни пациентов. Группа контроля – пациенты с ХОБЛ, не получавшие ДКТ, – была также составлена на основании ретроспективных данных.</p></sec><sec><title>Результаты</title><p>Результаты. Пятилетняя выживаемость пациентов с ХОБЛ, получавших ДКТ, составила 54,6 (95% ДИ 49,3–60,4) % (всего 324 чел., из них умерли 177, выжили 138, цензурированных 9), в группе контроля – 34,7 (95% ДИ 30,6–39,3) % (всего было 482 чел., из них умерли 298, выжили 166, цензурированных 18) (р &lt; 0,001).</p></sec><sec><title>Заключение</title><p>Заключение. Применение ДКТ положительно влияет на выживаемость пациентов с ХОБЛ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Chronic obstructive pulmonary disease (COPD) is a disease characterized by the persistent restriction of airflow and is a consequence of chronic inflammation of the respiratory tract and lung tissue in response to exposure to inhaled damaging particles or gases. The high prevalence of chronic obstructive pulmonary disease and the high risk of mortality among patients has long been an important problem of the modern healthcare. One of the most severe complications of COPD is chronic respiratory failure. The use of oxygen in patients with chronic hypoxemia should be constant, prolonged and as usually carried out at home; therefore, this form of therapy is called long-term oxygen therapy (LTOT).</p></sec><sec><title>Aim</title><p>Aim. To determine the effect of the use of LTOT on survival in patients with COPD and chronic respiratory failure.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 324 patients were retrospectively included in the study. They were hospitalized in the period from 2011 to 2017 with a confirmed diagnosis of COPD with FEV1 31,6% predicted used LTOT. Clinical data were obtained from patient medical histories. The control group (patients with COPD who did not receive LTOT) was also compiled based on retrospective data.</p></sec><sec><title>Results</title><p>Results. The 5-year survival rate of patients with COPD treated with LTOT was 54.6 (95% CI 49.3–60.4) % (a total of 324 people, 177 of them died, 138 survived, 9 people were censored), in the control group – 34.7 (95% CI 30.6–39.3) % (482 in total people, 298 of them died, 166 survived, 18 censored) (р &lt; 0.001).</p></sec><sec><title>Conclusions</title><p>Conclusions.
