Проблема ранних проявлений климактерического синдрома обусловлена высокой распространенностью среди женщин в постменопаузе, широким спектром клинических проявлений, резким снижением качества жизни, не всегда достаточной эффективностью менопаузальной гормональной терапии (МГТ) при наличии ограничений на ее проведение. Цель. Оценить динамику клинических проявлений климактерического синдрома и определить патогенетические эффекты препарата витамина D при использовании его в комплексе с МГТ у женщин раннего постменопаузального возраста с климактерическим синдромом.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовано 154 женщины, из которых 81 характеризовалась наличием клинических проявлений климактерического синдрома. Все женщины с климактерическим синдромом в течение 6 месяцев получали МГТ препаратом эстрадиола и дидрогестерона, при этом 39 женщин дополнительно также принимали препарат витамина D. До и после терапии у пациентов с климактерическим синдромом проводилась оценка распространенности симптомов шкалы Грина. В сыворотке крови всех женщин определяли концентрации 25(OH)D, остеопротегерина (OPG) и лиганда активатора рецептора ядерного фактора κВ (RANKL).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Использование МГТ женщинами с климактерическим синдромом приводит к снижению частоты регистрации ряда симптомов шкалы Грина (p &lt; 0,05). При этом уменьшение частоты регистрации отдельных жалоб обнаруживается только в группе женщин, которые наряду с МГТ получали препарат витамина D (p &lt; 0,05). Прием препарата витамина D сопровождается нарастанием исходно сниженных его сывороточных концентраций до нормальных значений (p &lt; 0,001). Добавление холекальциферола в комплексную терапию климактерического синдрома обеспечивает нормализацию уровня RANKL за счет снижения его продукции в динамике лечения (p &lt; 0,05). Уменьшение концентрации RANKL в ответ на комплексный прием гормонального препарата и холекальциферола обусловило к концу терапии более высокие значения индекса OPG/RANKL в основной группе, чем у женщин группы сравнения (р &lt; 0,01).</p></sec><sec><title>Выводы</title><p>Выводы. Полученные эффекты препарата витамина D при назначении его в комплексе с менопаузальной гормонотерапией (снижение частоты симптомов климактерического синдрома, уменьшение исходно повышенных уровней RANKL) свидетельствуют о целесообразности его использования в лечении ранних проявлений климактерического синдрома.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The problem of early manifestations of menopausal syndrome is due to the high prevalence among postmenopausal women, a wide range of clinical manifestations, a sharp decrease in the quality of life, not always sufficient effectiveness of menopausal hormone therapy (MHT) in the presence of restrictions on its use.</p></sec><sec><title>The aim</title><p>The aim. To evaluate the dynamics of clinical manifestations of menopausal syndrome and to determine the pathogenetic effects of vitamin D preparation when used in combination with menopausal hormone therapy in women of early postmenopausal age with menopausal syndrome.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 154 women were examined, of which 81 were characterized by the presence of clinical manifestations of menopausal syndrome. All women with menopausal syndrome received menopausal hormone therapy with estradiol and didrogesterone drug for 6 months, while 39 women additionally also took the vitamin D drug. Before and after therapy in patients with menopausal syndrome, the prevalence of symptoms of the Green scale was assessed. Concentrations of 25(OH) D, osteoprotegerin (OPG) and receptor activator of nuclear factor kappa-B ligand (RANKL) were determined in the blood serum of all women.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The use of MHT by women with menopausal syndrome leads to a decrease in the frequency of registration of a number of symptoms of the Green scale (p &lt; 0.05). At the same time, a decrease in the frequency of registration of individual complaints is found only in the group of women who, along with MYT, received a vitamin D preparation (p &lt; 0.05). Taking the vitamin D preparation is accompanied by an increase in its initially reduced serum concentrations to normal values (p &lt; 0.001). The addition of cholecalciferol to the complex therapy of menopausal syndrome ensures the normalization of RANKL levels by reducing its production in the dynamics of treatment (p &lt; 0.05) A decrease in the concentration of RANKL in response to the complex intake of a hormonal drug and cholecalciferol caused higher OPG/RANKL index values in the basic group by the end of therapy than in women of the comparison group (p &lt; 0.01).</p></sec><sec><title>Conclusions</title><p>Conclusions.
