Сахарный диабет – хроническое заболевание, протекающее с нарушением всех видов обмена веществ. Инкретиномиметики, такие как семаглутид, являются перспективной группой препаратов для лечения сахарного диабета 2-го типа как за счет увеличения эффективности контроля уровня гликемии, так и за счет дополнительного влияния на сердечно-сосудистую систему и массу тела человека. Разработка воспроизведенного препарата, содержащего семаглутид, является актуальной задачей, решение которой позволит увеличить доступность препаратов семаглутида в РФ.</p></sec><sec><title>Цель</title><p>Цель. Изучить сравнительную фармакокинетику, биоэквивалентность, безопасность и переносимость препаратов GP40221 и Оземпик®, содержащих семаглутид, у здоровых добровольцев.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Данное исследование биоэквивалентности являлось открытым рандомизированным и было проведено в параллельных группах здоровых добровольцев мужского пола с однократным введением исследуемых препаратов в дозе 0,5 мг натощак. Вывод о биоэквивалентности сравниваемых препаратов был сделан с использованием классического подхода, основанного на оценке 90%-ных доверительных интервалов для отношений средних геометрических значений первичных фармакокинетических параметров (AUC0-t, Сmax) для действующего вещества исследуемых препаратов.</p></sec><sec><title>Результаты</title><p>Результаты. По результатам исследования было показано, что значения 90% ДИ для отношений геометрических средних основных ФК-параметров семаглутида составляют 92,26–105,58% и 89,28–103,21% для AUC0-t и Cmax соответственно и укладываются в допустимые пределы 80–125%. Была доказана сопоставимая безопасность исследуемых препаратов семаглутида.</p></sec><sec><title>Выводы</title><p>Выводы. Таким образом, по результатам настоящего клинического исследования, препараты GP40221 (ООО «ГЕРОФАРМ», Россия) и Оземпик® (Ново Нордиск А/С, Дания) могут быть признаны биоэквивалентными и одинаково безопасными. Результаты данного исследования позволяют рекомендовать представление тестируемого препарата GP40221 в Министерство здравоохранения РФ для получения регистрационного удостоверения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Diabetes mellitus is a chronic disease that can impact all aspects of metabolism. Incretin mimetics, such as semaglutide, are a promising group of drugs to treat type 2 diabetes mellitus both through the improvement of glycemic control and additional effects on the cardiovascular system and body weight. The development of a generic semaglutide-containing drug is a burning issue which settlement will increase the availability of semaglutide in the Russian Federation</p></sec><sec><title>Aim</title><p>Aim. To study the comparative pharmacokinetics, bioequivalence, safety and tolerability of a semaglutide containing GP40221 and Ozempic® in healthy volunteers.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. This open-label, randomized, single-dose, parallel group study assessed the bioequivalence of a single dose of 0.5 mg of the study drugs in healthy male subjects under fasting conditions. The conclusion about the bioequivalence of the brand name drug versus the generic drug was made using the classical approach based on the assessment of 90% confidence intervals of the ratios of geometric means of the primary pharmacokinetic parameters (AUC0-t, Сmax) for the active substance of the study drugs.</p></sec><sec><title>Results</title><p>Results. The results of the study showed that the 90% CI values of the ratios of geometric means of the primary PK parameters of semaglutide were 85.96–109.01% and 89.14–111.40% for AUC0-t и Cmax, respectively, and are well within acceptable limits 80.00–125.00%. The comparable safety of the study drugs containing semaglutide has been proven.</p></sec><sec><title>Conclusion</title><p>Conclusion. Thus, GP40221 (GEROPHARM LLC, Russia) and Ozempic® (Novo Nordisk A/S, Denmark) can be considered bioequivalent and equally safe based on the results of this clinical study.
