Острая сенсоневральная тугоухость (ОСНТ) проявляется внезапным или быстрым снижением слуха без явных нарушений во внешнем и среднем ухе. Нередко причина этого состояния не устанавливается. На данный момент в лечении применяются различные методы, в т. ч. кортикостероиды, которые чаще всего используются в качестве первичной терапии. Недавние исследования демонстрируют эффективность местного введения стероидов.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность и безопасность различных способов топического и стандартного системного применения дексаметазона в лечении больных ОСНТ, а также определить оптимальные сроки терапии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проведенном исследовании был пролечен 61 пациент с этим заболеванием, 20 из них получали дексаметазон 4 мг/мл транстубарно, 21 – транстимпанально и 20 – системно. Аудиологическое исследование проводилось всем пациентам до лечения, при выписке, спустя месяц и полгода после выписки. Все участники регулярно оценивали свой дискомфорт по 10-балльной шкале.</p></sec><sec><title>Результаты</title><p>Результаты. Результаты лечения продемонстрировали у больных сенсоневральной тугоухостью эффективность как системной, так и местной стероидной терапии, однако на фоне топического использования препарата доля больных с полным восстановлением слуха была больше. Транстимпанальное введение стероидов после радиоволновой миринготомии оказалось наиболее комфортным для пациентов, уровень дискомфорта составил 1,5 баллов из 10, тогда как на фоне транстубарного введения препарата и системного лечения, соответственно, 4,5 и 2,3 балла.</p></sec><sec><title>Выводы</title><p>Выводы. Метод транстимпанального введения кортикостероидов у пациентов с ОСНТ после радиоволновой миринготомии является надежным и результативным способом доставки лекарства к окнам лабиринта. Данный подход позволяет визуально контролировать процесс введения медикамента в барабанную полость. Транстимпанальный и транстубарный методы введения стероидов по эффективности сопоставимы с классической системной стероидной терапией при лечении пациентов с ОСНТ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Sudden sensorineural hearing loss is manifested by sudden or rapid hearing loss without obvious disturbances in the outer and middle ear. Often the cause of this condition is not established. At the moment, various methods are used in the treatment, including corticosteroids, which are most often used as primary therapy. Recent studies demonstrate the effectiveness of topical steroid administration.</p></sec><sec><title>Aim</title><p>Aim. Assess the effectiveness and safety of various methods of topical and standard systemic use dexamethasone in the treatment of patients with acute sensorineural hearing loss, as well as to determine the optimal timing of therapy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 61 patients with this disease were treated in the study. 20 of them received dexamethasone transtubarly, 21 – transtympanic, and 20 – systemically. Audiological examination was performed on all patients before treatment, at discharge, a month and six months after discharge. All participants regularly assessed their discomfort on a 10-point scale. Results. The results of treatment demonstrated the effectiveness of both systemic and local steroid therapy in patients with sensorineural hearing loss; however, with topical use of the drug, the proportion of patients with complete hearing restoration was greater. Transtympanic introduction of steroids after radiowave myringotomy turned out to be the most comfortable for patients the level of discomfort was 1.5 points out of 10, while against the background of transtubal administration of the drug and systemic treatment it was 4.5 and 2.3 points, respectively.</p></sec><sec><title>Conclusions</title><p>Conclusions. The method of transtympanic administration of corticosteroids in patients with acute sensorineural hearing loss after radiowave myringotomy is a reliable and effective method of drug delivery to the windows of the labyrinth. This approach allows you to visually control the process of introducing the medication into the tympanic cavity.
