Хроническая головная боль напряжения и инсомния – частая взаимоусугубляющая патология. Препараты, применяемые для лечения этих нозологий, чаще всего ухудшают течение сопутствующего заболевания.</p></sec><sec><title>Цель</title><p>Цель. Оценить влияние коррекции инсомнии на интенсивность и частоту головной боли.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследовании приняли участие 27 пациентов (25 женщин, 2 мужчин) с хронической головной болью напряжения и нарушениями сна. Медианный возраст составил 43 ± 14,8 года. В соответствии с целью исследования выделены 2 группы. В 1-й проведено лечение венлафаксином и доксиламином, во 2-й группе – только лечение венлафаксином. Оценка проводилась до начала лечения и через 8 нед. после с применением визуальных аналоговых шкал головной боли, сна, состояния здоровья: оценка тяжести инсомнии по шкалам Морин и Питтсбургский опросник качества сна (PSQI), оценка эмоционального состояния по шкалам тревоги Шихана и депрессии Бека и полисомнография.</p></sec><sec><title>Результаты</title><p>Результаты. В 1-й группе отмечалось достоверное снижение количества дней с головной болью (с 20 до 10, p = 0,002), интенсивности боли по ВАШ с 7 до 6 баллов (p = 0,01) и ВАШ сна с 6 до 4 баллов (p = 0,003). Во 2-й группе достоверно снижалось число дней с головной болью (с 15 до 12, p = 0,01), изменения в выраженности головной боли по ВАШ с 7,5 до 6 баллов (p = 0,13) и качеству сна с 6 до 7,5 балла (p = 0,38), не имели статистически значимой разницы. По результатам полисомнографии, у пациентов 1-й группы отмечалось увеличение продолжительности медленноволнового сна на 12 мин. (p = 0,02), в то время как у пациентов из 2-й группы на 4 мин. (p = 0,68).</p></sec><sec><title>Заключение</title><p>Заключение. Применение комбинации венлафаксина и доксиламина на первых этапах лечения пациентов с хронической головной болью напряжения и инсомнией имеет преимущество перед приемом венлафаксина. Комбинированная терапия позволяет снизить интенсивность и частоту головной боли и улучшить качество сна.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Chronic tension headache and insomnia are a common mutually aggravating pathology. Drugs used to treat these nosologies most often worsen the course of the concomitant disease.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the effect of insomnia correction on the intensity and frequency of headaches</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study involved 27 patients (25 women, 2 men) with chronic tension-type headache and sleep disorders. The median age was 43 ± 14.8 years. In accordance with the purpose of the study, the clinical group was divided into 2 subgroups. The first group was treated with venlafaxine and doxylamine, the patients of the second group received only venlafaxine treatment. The assessment was carried out before the start of treatment and 8 weeks after, using visual analogue scales for headache, sleep, health status, assessing the severity of insomnia using the Morin scale and the Pittsburgh Sleep Quality Inventory (PSQI), assessing the emotional state using the Sheehan anxiety and Beck depression scales and polysomnography.</p></sec><sec><title>Results</title><p>Results. In subgroup 1, which received correction of insomnia, there was a significant decrease in the number of days with headache (from 20 to 10, p = 0.002), pain intensity according to VAS (from 7 to 6, p = 0.01) and VAS sleep (from 6 to 4 points, p = 0.003). In the group without correction of insomnia, the number of days with headaches significantly decreased (from 15 to 12, p = 0.01), changes in the severity of headache according to VAS (from 7.5 to 6, p = 0.13) and sleep quality (from 6 to 7.5, p = 0.38) did not have a statistically significant difference. According to the results of polysomnography, patients who received correction of insomnia had an increase in the duration of slow-wave sleep by 12 minutes (p = 0.02), while in patients from the second subgroup by 4 minutes (p = 0.68).</p></sec><sec><title>Conclusions</title><p>Conclusions. The use of a combination of venlafaxine and doxylamine in the first stages of treatment of patients with chronic tension-type headache and insomnia has an advantage over standard therapy.
