Оптимальная первая линия терапии пациентов с гормоноположительным (HR+) HER2-отрицательным (HER2–) метастатическим раком молочной железы (мРМЖ) включает использование ингибиторов CDK4/6 в сочетании с эндокринной терапией. Комбинированная эндокринотерапия доказала свою эффективность в многочисленных рандомизированных контролируемых исследованиях (РКИ) II–III фазы. Однако ввиду строгих критериев включения РКИ не могут полностью охватить все возможные клинические ситуации, что приводит к сосредоточению внимания на определенной популяции пациентов.</p></sec><sec><title>Цель</title><p>Цель. Сделать анализ данных реальной клинической практики по использованию рибоциклиба для лечения пациентов с HR+ HER2– мРМЖ в Республиканском клиническом онкологическом диспансере (Уфа).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены пациенты с диагнозом HR+ HER– мРМЖ, которые в период с 2016 по 2020 г. в рутинной практике получали рибоциклиб в комбинации с эндокринной терапией. Данные о клинико-патологических характеристиках пациентов и прогрессировании заболевания при приеме рибоциклиба были получены путем анализа клинических данных историй заболевания и отчетов о радиологических/патоморфологических исследованиях.</p></sec><sec><title>Результаты</title><p>Результаты. Были проанализированы данные о 101 пациентке, медиана возраста составила 61 год. В исследование были включены 67 (66,3%) пациенток с прогрессированием после радикального лечения и 34 (33,7%) пациентки с первично-диссеминированным РМЖ. Снижение дозы из-за нежелательных явлений было произведено в 14 (13,9%) случаях. Наличие предшествующих линий терапии и более низкий статус ECOG были связаны с более короткой выживаемостью без прогрессирования (ВБП), тогда как люминальный А-подтип опухоли и выбор фулвестранта в качестве эндокринного препарата-партнера были связаны с более продолжительной ВБП. Характер метастазирования, положительная экспрессия рецепторов прогестерона и снижение дозы из-за токсичности не оказали значимого влияния на ВБП.</p></sec><sec><title>Заключение</title><p>Заключение. Настоящий анализ реальных данных о применении рибоциклиба в российской региональной больнице подтверждает данные РКИ относительно эффективности и безопасности использования ингибиторов CDK4/6 для лечения пациентов с HR+ HER2– мРМЖ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The standard initial treatment for patients with hormone receptor positive, HER2 negative, metastatic breast cancer (HR+/HER2– mBC) involves the use of CDK4/6 inhibitors (CDK4/6i) in combination with endocrine therapy. The combination therapy has proven effectiveness in numerous Phase II and III randomized controlled trials (RCTs). Nonetheless, RCTs may not fully represent real-world clinical situations due to their stringent inclusion criteria, resulting in a specific patient population.</p></sec><sec><title>Aim</title><p>Aim. Analyze of ribociclib using for treating patients with HR+/HER2– advanced breast cancer (mBC) at the Republican Clinical Oncology Dispensary.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Patients diagnosed with HR+/HER2– mBC who were treated in clinical routine with ribociclib in combination with different endocrine agents between 2016 and 2020 were identified and enrolled in this retrospective study. Clinicopathological characteristics and clinical outcomes were recorded with particular emphasis on ribociclib therapy course (progression-free survival (PFS), toxicity, dose reduction).</p></sec><sec><title>Results</title><p>Results. Data from n = 101 patients were evaluated. The mean patient age was 61 years. The study included 67 (66.3%) patients with progression after radical treatment and 34 (33.7%) patients with primary disseminated breast cancer. Dose reduction due to adverse events was performed in n = 14 cases (13.9%). Presence of prior therapy lines and lower ECOG status were associated with shorter PFS, whereas estrogen positivity and the choice of fulvestrant over aromatase inhibitor were positively associated with PFS. Metastatic pattern, progesterone positivity and dose reduction due to toxicity did not significantly impact on PFS.</p></sec><sec><title>Conclusion</title><p>Conclusion. Our real-world data analysis on ribociclib treatment in Russian regional hospital supports data from RCTs regarding both treatment efficacy and safety of CDK4/6i for treatment of patients with HR+/HER2– mBC.
