Данная статья посвящена детальному разбору клинического случая пациентки Т. 2009 года рождения с установленным диагнозом «бронхиальная астма, атопическая форма, тяжелое персистирующее частично-контролируемое течение», которой была инициирована терапия генно-инженерным биологическим препаратом гуманизированного моноклонального антитела против IgE – омализумабом. Однако в последующие годы состояние пациентки было нестабильным, тяжелые приступы повторялись, неоднократно она была госпитализирована в отделение, где проводилась инфузионная терапия и пересматривалась базисная терапия, проводились коррекции дозы и схемы введения моноклональных антител к IgE (омализумаб). Также был проведен ретроспективный анализ результатов объективного, инструментального и лабораторного обследований больной на протяжении последних 3 лет, когда ребенок без перерыва получал генно-инженерный препарат омализумаб. Принимая к сведению тяжелое течение БА, отсутствие контроля над заболеванием на фоне базисной терапии комбинированными препаратами в сочетании с терапией моноклональными антителами к IgE (омализумаб), было принято решение о коррекции лечения и инициации терапии генно-инженерным препаратом, рекомбинантным человеческим моноклональным антителом IgG4 (дупилумаб). Данный клинический пример еще раз показывает, насколько персонифицированным должен быть подход при назначении терапии пациентам с тяжелым течением БА, а также диктует необходимость разработки новых методов диагностики и тактик ведения больных с неконтролируемыми формами данного заболевания.</p></abstract><trans-abstract xml:lang="en"><p>Currently, bronchial asthma is a global health problem, due to its high prevalence, economic component, as well as a violation of the social adaptation of children suffering from this disease. This article is devoted to a detailed analysis of the clinical case of a patient born in 2009 with an established diagnosis of Bronchial asthma, atopic form, severe persistent partially controlled course, which initiated therapy with a genetically engineered biological preparation of a humanized monoclonal antibody against IgE – omalizumab. However, in subsequent years, the patient’s condition was unstable, severe seizures were repeated, she was repeatedly hospitalized in the department where infusion therapy was performed and basic therapy was reviewed, dose adjustments and administration regimens of monoclonal antibodies to IgE (omalizumab) were carried out. A retrospective analysis of the results of objective, instrumental and laboratory examinations of the patient was also carried out over the past three years, when the child received the genetically engineered drug omalizumab without interruption. Taking into account the severe course of AD, the lack of control over the disease against the background of basic therapy with combined drugs in combination with therapy with monoclonal antibodies to IgE (omalizumab), it was decided to correct treatment and initiate therapy with a genetically engineered drug, recombinant human monoclonal antibody IgG4 (dupilumab).
