Такие заболевания, как атопический дерматит (АтД) и ихтиоз, связаны с генетическими мутациями генов эпидермальной дифференцировки, при других заболеваниях (контактные дерматиты, экзема) нарушения эпидермального барьера возникают в результате воспалительного процесса в коже, механического или химического повреждения, значительно влияя на течение патологического процесса. В статье освещены современные данные о значении основного структурного белка рогового слоя филаггрина (FLG) и роли его дефицита не только при дерматологических заболеваниях, но и в формировании гиперреактивности. Основными способами коррекции нарушений кожного барьера является применение эмолентов, которые относятся к методам базисной терапии при АтД и рекомендуются к применению в комплексной терапии других патологических состояний, сопровождающихся явлениями ксероза. На сегодняшний день разработан инновационный метод, позволяющий не только замещать дефицит FLG, но и активировать собственные ресурсы и стимулировать синтез белка в коже. В связи с этим представляет интерес уникальное средство дерматокосметики Адмера, относящееся к категории «эмоленты плюс» за счет сочетания в себе всех необходимых свойств эмолента и наличия модулятора синтеза FLG филагринола. Филагринол представляет собой запатентованный комплекс активных веществ, которые активируют ферменты, участвующие в дефосфорилировании предшественника FLG профилаггрина, и увеличивают концентрацию в клетках зернистого слоя гликопротеина, богатого гистидином, участвующим в формировании цитоскелета рогового слоя. В статье представлен обзор клинических исследований по эффективности крема Адмера и приводится собственный клинический опыт его применения у пациентов с АтД и экземой кистей.</p></abstract><trans-abstract xml:lang="en"><p>Skin xerosis is a common symptom that indicates of impaired skin barrier function. Such diseases as atopic dermatitis (AD) and ichthyosis are associated with genetic mutations of epidermal differentiation genes, while in other diseases (contact dermatitis, eczema) epidermal barrier disorders occur as a result of inflammatory process in the skin, mechanical or chemical damage, significantly affecting the course of the pathological process. The article highlights modern data on the importance of the main structural protein of the stratum corneum filaggrin (FLG) and the role of its deficiency not only in dermatologic diseases, but also in the development of hyperresponsiveness. The main methods of correction of skin barrier disorders are emollients, which belong to the methods of basic therapy in AD and are recommended for the complex therapy of other pathological conditions accompanied by xerosis. At the present time, an innovative method has been developed, that allows not only to replace FLG deficiency, but also to activate and stimulate protein synthesis in the skin. In this regard, a unique dermatocosmetics product “Admera”, which belongs to the category of “emollients plus” due to the combination of all the necessary properties of the emollient and the presence of FLG synthesis modulator filagrinol, is of interest. Filagrinol is a proprietary complex of active ingredients that activate enzymes involved in dephosphorylation of the FLG predecessor profilaggrin and increase the concentration in the stratum granulosum сells of a histidine-rich glycoprotein, involved in the formation of the stratum corneum cytoskeleton.
