Атопический дерматит (АтД) является одним из самых распространенных дерматитов, характеризующихся сложными патогенетическими механизмами. Одним из триггеров АтД признан психологический стресс. Стресс вызывает высокий выброс кортизола и адреналина или норадреналина, стимулируя иммунную систему, в первую очередь Т-хелперы типа 1 (клетки Th1), для выработки провоспалительных цитокинов, что приводит к клеточному иммунному ответу и воспалению. В последние годы отмечается рост заболеваемости среди беременных женщин, однако особенности механизмов развития АтД при беременности до сих пор остаются малоизученными.</p></sec><sec><title>Цель</title><p>Цель. Изучить роль кортизола при АтД во время беременности.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включили 76 беременных женщин во время обострения АтД, 20 небеременных женщин во время обострения АтД, 20 небеременных женщин без АтД, 20 беременных женщин без АтД. Степень тяжести АтД определялась с помощью индекса SCORAD. Уровень кортизола определялся в сыворотке крови методом иммуноферментного анализа (ИФА). Уровень тревоги определялся с использованием шкалы тревоги Бека. Уровень зуда определялся с использованием шкалы зуда 5D.</p></sec><sec><title>Результаты</title><p>Результаты. Уровень кортизола у беременных женщин с АтД (629,8 пг/мл) значительно выше, чем у небеременных женщин с АтД (386,15 пг/мл) (р &lt; 0,05). Определены корреляционные связи уровня кортизола со степенью тяжести (коэффициент Спирмена = 0,203, р = 0,018), уровнем тревоги (коэффициент Спирмена = 0,411, р = 0,001), уровнем зуда (коэффициент Спирмена = 0,352, р = 0,001).</p></sec><sec><title>Выводы</title><p>Выводы. Кортизол имеет важное значение в патогенезе АтД при беременности. При беременности с обострением АтД отмечены более высокие значения, чем вне беременности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Atopic dermatitis (AD) is one of the most common dermatitis, characterized by complex pathogenetic mechanisms. Psychological stress is recognized as one of the triggers of AD. Stress causes a high release of cortisol and epinephrine or norepinephrine, stimulating the immune system, primarily T helper cells type 1 (Th1 cells), to produce pro-inflammatory cytokines, leading to a cellular immune response and inflammation. In recent years, there has been an increase in incidence among pregnant women, however, the specific mechanisms of the development of AD during pregnancy still remain poorly understood. Aim. To study the role of cortisol in AD during pregnancy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 76 pregnant women during an exacerbation of AD, 20 non-pregnant women during an exacerbation of AD, 20 non-pregnant women without AD, 20 pregnant women without AD. The severity of AD was determined using the SCORAD index. Cortisol levels were determined in blood serum using enzyme-linked immunosorbent assay (ELISA). Anxiety level was determined using the Beck Anxiety Inventory. The level of itching was determined using a 5D itching scale.</p></sec><sec><title>Results</title><p>Results. Cortisol levels in pregnant women with AD (629.8 pg/ml) were significantly higher than in non-pregnant women with AD (386.15 pg/ml) (p &lt; 0.05). Cortisol levels were correlated with the severity level (Spearman coefficient = 0.203, p = 0.018), anxiety level (Spearman coefficient = 0.411, p = 0.001), and level of itching (Spearman coefficient = 0.352, p = 0.001).</p></sec><sec><title>Conclusions</title><p>Conclusions. Cortisol is important in the pathogenesis of AD during pregnancy.
