Поиск маркеров, отражающих иммунные нарушения, и их вклад в прогрессирование цирроза печени (ЦП) является весьма актуальным.</p></sec><sec><title>Цель</title><p>Цель. Изучить возможность использования показателя «отношение нейтрофилов к лимфоцитам» в качестве предиктора развития синдрома системного воспалительного ответа и летального исхода.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Для ретроспективного клинического исследования отобраны 225 историй болезней пациентов с ЦП в период с 2008 по 2018 г. Из них сформированы три группы: 1-я – пациенты с ЦП класса А по Чайлд – Пью (n = 24); 2-я – пациенты с ЦП класса В и С по Чайлд – Пью (n = 201) и 3-я – здоровые лица (n = 50). Проанализирована прогностическая ценность показателя «отношение нейтрофилов к лимфоцитам» (ОНЛ) в развитии синдрома системного воспалительного ответа (ССВО) и летального исхода у пациентов с ЦП.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов с ЦП отмечались статистически значимо большие значения ОНЛ по сравнению со здоровыми лицами (p &lt; 0,001). Показатель ОНЛ – независимый фактор риска и статистически значимый предиктор развития ССВО у пациентов с ЦП. Значение ОНЛ &gt; 3,59 характеризовалось чувствительностью 0,38 [95% ДИ: 0,28; 0,48] и специфичностью 0,93 [95% ДИ: 0,87; 0,97]. ОНЛ служило статистически значимым предиктором развития летального исхода (p &lt; 0,001). Значение ОНЛ &gt; 4,5 характеризовалось чувствительностью 0,24 [95% ДИ: 0,15; 0,36] и специфичностью 0,97 [95% ДИ: 0,92; 0,99].</p></sec><sec><title>Выводы</title><p>Выводы. Значение ОНЛ более 3,59 повышает риск развития ССВО у пациентов с ЦП, а значение ОНЛ более 4,5 – риск развития летального исхода.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The search for markers reflecting immune disorders and their contribution to the progression of liver cirrhosis (LC) is very relevant.</p></sec><sec><title>Aim</title><p>Aim. To study the possibility of using the “neutrophil to lymphocyte ratio” indicator as a predictor of the development of systemic inflammatory response syndrome and death.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. For a retrospective clinical study, 225 case histories of patients with cirrhosis were selected from 2008 to 2018. Three groups were formed from them: group 1: patients with cirrhosis class A according to Child – Pugh (n = 24); group 2: patients with cirrhosis class B and C according to Child – Pugh (n = 201) and group 3: healthy individuals (n = 50). The prognostic value of the neutrophil-to-lymphocyte ratio (NLR) indicator in the development of systemic inflammatory response syndrome (SIRS) and death in patients with cirrhosis was analyzed.</p></sec><sec><title>Results</title><p>Results. Patients with LC had statistically significantly higher values of NLR compared to healthy individuals (p &lt; 0.001). The NLR indicator is an independent risk factor and a statistically significant predictor of the development of SIRS in patients with LC. A NLR value &gt; 3.59 had a sensitivity of 0.38 [95% CI: 0.28; 0.48] and specificity 0.93 [95% CI: 0.87; 0.97]. NLR served as a statistically significant predictor of death (p &lt; 0.001). A NLR value &gt; 4.5 had a sensitivity of 0.24 [95% CI: 0.15; 0.36] and specificity 0.97 [95% CI: 0.92; 0.99].</p></sec><sec><title>Conclusion</title><p>Conclusion.
