Болезни органов дыхания занимают одно из лидирующих мест в структуре общей заболеваемости населения Российской Федерации. Основным осложнением течения респираторных заболеваний является дыхательная недостаточность, для лечения которой применяется длительная кислородотерапия и респираторная поддержка: неинвазивная вентиляция легких и искусственная вентиляция легких. Зарубежные исследования свидетельствуют о широкой распространенности использования респираторной поддержки и кислородотерапии на дому (РПиК на дому) у пациентов, нуждающихся в т. ч. в паллиативной медицинской помощи.</p></sec><sec><title>Цель</title><p>Цель. Оценить клиническую эффективность применения технологии РПиК на дому.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование было включено 98 пациентов с хронической дыхательной недостаточностью. Пациентам применяли медицинскую технологию РПиК на дому согласно протоколу исследования. В зависимости от типа РПиК на дому все пациенты были разделены на три группы: длительной кислородотерапии - 47 человек, неинвазивной вентиляции легких - 43, инвазивной вентиляции легких - 8. Оценивались параметры пациентов на инициирующем визите и трех последующих. Пациенты наблюдались в течение 9 мес.</p></sec><sec><title>Результаты</title><p>Результаты. В ходе исследования было отмечено статистически значимое уменьшение выраженности симптомов ХДН (значение шкал САТ и mMRC) и улучшение качества жизни и общего состояния пациента (значение опросника SF-36 и шкале Карновского). В ходе исследования 38 больным потребовалась госпитализация: из них у 8 госпитализация была связана с обострением гиперкапнической ОДН. Частота благополучных исходов составила 87 случаев.</p></sec><sec><title>Выводы</title><p>Выводы. Исследование показало, что РПиК на дому является эффективной, безопасной и клинически обоснованной технологией для больных с ХДН, проведение которой уменьшает проявление клинических симптомов, уменьшает число госпитализаций, снижает летальность, улучшает качество жизни, а также демонстрирует эффективность применения респираторного оборудования на дому, а также телемедицинских технологий.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Respiratory diseases are number one in the structure of general morbidity of the population of the Russian Federation. The main complication of respiratory diseases is respiratory failure, which may be treated with long-term oxygen therapy (LTOT) and respiratory support (RS): non-invasive mechanical lung ventilation (NIV) and invasive mechanical lung ventilation (IV). International medical experience indicates a widespread enough implementation of respiratory support and oxygen therapy at home (RS and OT at home) for patients including whose requiring palliative medical care.</p></sec><sec><title>Aim</title><p>Aim. The purpose of the study was to evaluate the clinical results of implementation of respiratory support and oxygen therapy at home.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 98 patients with chronic respiratory failure (CRF). All patients used RS and OT at home according to the Study Protocol. All patients were divided into 3 groups: LTOT - 47 patients, NIV in combination or with-out LTOT - 43 patients, IV in combination or without LTOT - 8 patients. The estimated parameters of the patients' condition were measured at the initiating visit and three subsequent control sessions. The planned duration of the Study was 9 months.</p></sec><sec><title>Results</title><p>Results. During the Study, a statistically significant decrease in the severity of symptoms of respiratory failure was noted (CAT and mMRC scales) as well as an improvement in the quality of life and the general condition of the patient (SF-36 questionnaire and Karnovsky scale). During the study, 38 patients required hospitalization: in 8 cases hospitalization was associated with an exacerbation of CRF. The number of successful outcomes was 87 cases.</p></sec><sec><title>Conclusion</title><p>Conclusion.
