В статье рассмотрены подходы к лечению пациентов с III стадией немелкоклеточного рака легкого (НМРЛ). Особое внимание уделяется неоадъювантной и адъювантной лекарственной терапии.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность неоадъювантных методов лечения, таких как неоадъювантная химио- и иммунохимиотерапия, и адъювантных подходов к терапии НМРЛ III стадии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Результаты были получены в ходе исследования - клиническая апробация рака легкого («КАРЛ- 001»). В исследование было включено 186 нелеченных пациентов с НМРЛ IIIA-IIIC стадии (160 пациентов с IIIA стадией, 25 с IIIB и 1 с IIIC). Включено 108 пациентов с аденокарциномой и 78 с плоскоклеточным раком. Операция с адъювантной полихимиотерапией (аПХТ) была выполнена 118 пациентам, неоадъюватная полихимиотерапия (неоПХТ) с последующей операцией - 49 пациентам, а неоадъювантная иммунохимиотерапия (неоИХТ) - 19 пациентам. Медиана наблюдения составила 40 мес.</p></sec><sec><title>Результаты</title><p>Результаты. Применение как адъювантной, так и неоадъювантной химиотерапии продемонстрировало одинаковую эффективность. Медиана безрецидивной выживаемости (БРВ) в группах аПХТ и неоПХТ составила 30,4 и 32,6 мес. соответственно (статистически значимо не различались). Общая выживаемость (ОВ) также статистически значимо не различалась. Применение неоИХТ показало лучшие результаты по сравнению с неоПХТ. В группе неоИХТ медиана БРВ достигнута не была, в группе неоПХТ медиана БРВ составила 32,6 мес. ОВ была нумерологически выше в группе неоИХТ по сравнению с неоПХТ – на период наблюдения в 36 мес. составила 78,9 и 59,18% соответственно.</p></sec><sec><title>Выводы</title><p>Выводы. Предоперационная и послеоперационная химиотерапия могут рассматриваться как равноценные варианты в лечении пациентов с III стадией НМРЛ. Применение неоИХТ продемонстрировало лучшие результаты по сравнению с неоПХТ. Дальнейшее изучение данных методов лечения позволит более персонализированно подходить к лечению пациентов с НМРЛ III стадии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The article considers approaches to the treatment of patients with stage III non-small-cell lung cancer (NSCLC). Particular attention is paid to neoadjuvant and adjuvant drug therapy.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the efficacy of neoadjuvant treatments, such as neoadjuvant chemotherapy and immunochemotherapy, and adjuvant approaches to the stage III NSCLC treatment.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The results were obtained during the study: Clinical Testing of Lung Cancer (“CARL-001”). A total of 186 untreated patients with stage IIIA-IIIC NSCLC (160 patients with stage IIIA, 25 patients with IIIB and 1 with IIIC) were enrolled in the study. The study included 108 patients with adenocarcinoma and 78 with squamous cell carcinoma. 118 patients underwent surgery with adjuvant polychemotherapy (aPCT), 49 patients underwent neoadjuvant polychemotherapy (neoPCT) followed by surgery, and 19 patients had neoadjuvant immunotherapy combined with chemotherapy (neoICT). The median follow-up was 40 months.</p></sec><sec><title>Results</title><p>Results. Both adjuvant and neoadjuvant chemotherapy shared equivalent efficacy. The median relapse-free survival (RFS) in the aPCT and neoPCT groups was 30.4 and 32.6 months, respectively (differences between groups were not statistically significant). There was no significant difference in overall survival (OS) either. The use of neoICT showed better results as compared to neoPCT. The median RFS was not reached in the neoICT group, the median RFS in the neoPCT group was 32.6 months. OS was numerically higher in the neoICT group as compared to the neoPCT group, and accounted for 78.9% and 59.18% over a 36-month follow-up period, respectively.</p></sec><sec><title>Conclusions</title><p>Conclusions. Preoperative chemotherapy is proven to be equal to the postoperative chemotherapy in the treatment of patients with stage III NSCLC. The use of neoICT demonstrated better results as compared to neoPCT.
