Систематизация дерматоскопических признаков мономорфных высыпаний при мастоцитозе имеет важное практическое значение.</p></sec><sec><title>Цель</title><p>Цель. Провести анализ дерматоскопических паттернов мономорфных высыпаний при мономорфном пятнисто-папулезном кожном мастоцитозе (ППКМ) и индолентном системном мастоцитозе (ИСМ) и сопоставить полученные результаты с патоморфологическими признаками мастоцитоза.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 6 детей в возрасте от 4 до 14 лет, 27 подростков и взрослых пациентов в возрасте от 15 до 40 лет с диагнозом «пятнисто-папулезный кожный мастоцитоз, мономорфный тип» и 3 пациента (минимальный возраст 18 лет, максимальный – 54 года) с диагнозом «системный индолентный мастоцитоз», находившихся под наблюдением в ГБУЗ «Московский центр дерматовенерологии и косметологии» с 2022 по 2024 г. Дерматоскопическое исследование проводилось с использованием поляризационной и иммерсионной дерматоскопии с 20-кратным увеличением.</p></sec><sec><title>Результаты</title><p>Результаты. Характерными особенностями моноППКМ у детей явились наличие слабовыраженных сосудистых признаков (точечные сосуды) и отсутствие пигментной псевдосети. У подростков и взрослых наличие пигментной сети было выявлено у 77,8%, чаще у мужчин. Сосудистый эритематозный компонент у взрослых был выражен интенсивнее, чем у детей, и выявлялся у 74,1% пациентов. Желто-коричневые бесструктурные области, коричневые ретикулярные линии (пигментная псевдосеть) обусловлены гиперпигментацией базального слоя клеток, в то время как сосудистый рисунок был результатом расширения дермальных сосудов в сосочковом слое дермы. При ИСМ наблюдались выраженный сосудистый паттерн (интенсивный эритематозный фон и ретикулярный сосудистый рисунок) и обязательное присутствие пигментной псевдосети. Патоморфологическая картина ИСМ отличалась от ППКМ присутствием периваскулярных лимфомоноцитарных инфильтратов и незначительного числа эозинофильных гранулоцитов.</p></sec><sec><title>Выводы</title><p>Выводы. В настоящем исследовании впервые были систематизированы возрастные дерматоскопические особенности мономорфных высыпаний при мастоцитозе, а также проведено сопоставление этих особенностей с патоморфологическими признаками ППКМ и ИСМ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Systematization of dermatoscopic signs of monomorphic rashes in mastocytosis has important practical significance.</p></sec><sec><title>Aim</title><p>Aim. To carry out an analysis of dermatoscopic patterns of monomorphic eruption in monomorphic maculopapular cutaneous mastocytosis (mMPCM) and indolent systemic mastocytosis (ISM) and compare the results obtained with pathomorphological signs of mastocytosis.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 6 children aged 4 to 14 years, 27 adolescents and adult aged 15 to 40 years with a monomorphic type of maculopapular cutaneous mastocytosis (MPCM) and 3 patients (minimum age 18 years, maximum – 54 years) with indolent systemic mastocytosis (ISM), who were under observation at Moscow scientific and practical Center of dermatovenereology and cosmetology from 2022 to 2024. Dermatoscopic examination was performed using polarization and immersion dermatoscopy with 20-fold magnification.</p></sec><sec><title>Results</title><p>Results. The presence of mild vascular signs (punctate vessels) and the absence of a pigment pseudonetwork were the features of monoMPCM in children. In adolescents and adults, pigment network was detected in 77.8%, more often in men. The vascular erythematous component in adults was more intense than in children and was detected in 74.1% of patients. Yellow-brown structureless areas, brown reticular lines (pigment pseudonetwork) are due to hyperpigmentation of the basal cell layer, while the vascular pattern was the result of dilation of vessels in the papillary dermis. With ISM, a pronounced vascular pattern (intense erythematous background and reticular vascular pattern) and the obligatory presence of pseudonetwork were observed. The pathomorphological picture of ISM differed from MPCM by the presence of perivascular lymphomonocytic infiltrates and an insignificant number of eosinophilic granulocytes.</p></sec><sec><title>Conclusion</title><p>Conclusion.
