CYP2D6 участвует в метаболизме тамсулозина, который используется для лечения симптомов нижних мочевых путей при доброкачественной гиперплазии предстательной железы (ДГПЖ). Однако влияние носительства полиморфных вариантов гена CYP2D6 на клиническую эффективность и безопасность терапии тамсулозином до сих пор остается недостаточно изученным.</p></sec><sec><title>Цель</title><p>Цель. Оценить влияние носительства аллельных вариантов гена CYP2D6 на остаточную равновесную концентрацию тамсулозина и корреляцию с эффективностью и безопасностью терапии у пациентов с симптомами нижних мочевых путей/ доброкачественной гиперплазией предстательной железы.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В одноцентровом проспективном исследовании приняли участие 75 пациентов, завершившие все этапы исследования. Медианный возраст пациентов составил 69 лет, индекс массы тела – 27,17 ± 4,86 кг/м². У 56 пациентов (74,67%) были сопутствующие заболевания, что означает наличие по крайней мере одного хронического заболевания в дополнение к диагнозу ДГПЖ. 26 человек (34,67%) были отнесены к полиморбидным пациентам, т. е. имеющим сразу несколько сопутствующих заболеваний, в то время как у 19 пациентов (25,33%) не было сопутствующих нозологий помимо ДГПЖ. Генотипирование было проведено методом полимеразной цепной реакции в реальном времени. Cssmin тамсулозина определяли с помощью высокоэффективной жидкостной хроматографии с масс-спектрометрическим детектированием. Эффективность терапии оценивали по изменению баллов по шкале IPSS и Qmax. Безопасность оценивалась на основе частоты побочных реакций.</p></sec><sec><title>Результаты</title><p>Результаты. Было установлено, что носительство CYP2D6*4 и CYP2D6*10 ассоциировалось со статистически значимым повышением уровня Cssmin. Однако выявленные фармакокинетические различия, носительство маркеров CYP2D6 не отражались на клинической эффективности и безопасности тамсулозина.</p></sec><sec><title>Выводы</title><p>Выводы. Носительство CYP2D6*4 и CYP2D6*10 может влиять на уровень Cssmin препарата, но не влияет на клинические результаты лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. CYP2D6 is involved in the metabolism of tamsulosin, which is used to treat lower urinary tract symptoms (LUTS) in benign prostatic hyperplasia (BPH). However, the impact of carrying polymorphic variants of the CYP2D6 gene on the clinical efficacy and safety of tamsulosin therapy is still not well understood.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the impact of CYP2D6 polymorphic variants on steady state concentration of tamsulosin (Cssmin), as well as their correlation with the efficacy and safety of therapy in patients with BPH.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A single-center prospective study included 75 patients who completed all stages of the study. The median age of the patients was 69 years, body mass index was 27.17 ± 4.86 kg/m². 56 patients (74.67%) had comorbidities, which means the presence of at least one chronic disease in addition to the diagnosis of benign prostatic hyperplasia. 26 people (34.67%) were classified as polymorbid patients, i.e. having several comorbidities at once, while 19 patients (25.33%) had no comorbidities except for benign prostatic hyperplasia. Genotyping was performed by real-time polymerase chain reaction. Cssmin of tamsulosin was determined by HPLC-MS/MS. Efficacy of therapy was assessed using IPSS score and Qmax. Safety was assessed based on the incidence of adverse reactions.</p></sec><sec><title>Results</title><p>Results. Carriage of CYP2D6*4 and CYP2D6*10 was found to be associated with statistically significant increases in Cssmin levels. However, the identified pharmacokinetic differences, carriage of CYP2D6 markers was not reflected in the clinical efficacy and safety of tamsulosin.</p></sec><sec><title>Conclusion</title><p>Conclusion.
