Применение поли-L-молочной кислоты (PLLA) оказывает многофакторное воздействие на инволюционные изменения кожи, способствуя увеличению содержания основных структурных элементов внеклеточного матрикса. Высокомолекулярная гиалуроновая кислота (ГК) обладает способностью к обеспечению высокого уровня увлажненности кожи и ингибированию выработки матриксных металлопротеиназ. Сочетание PLLA и ГК представляется перспективным с учетом синергизма их действия.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность и безопасность гибридного протокола одномоментного применения PLLA и высокомолекулярной ГК при инволюционных изменениях кожи с использованием ультразвукового исследования (УЗИ) и гистологической диагностики.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. PLLA в стандартном разведении вводили на глубину 3–4 мм в области передней брюшной стенки 6 здоровым женщинам с последующим внутрикожным введением ГК на глубину 2–3 мм. Эффективность и безопасность оценивали через 24 ч и 6 нед. на основе визуального осмотра, фотодокументирования, УЗИ и гистологического исследования.</p></sec><sec><title>Результаты</title><p>Результаты. Через 6 нед. после процедуры наблюдалось уплотнение и выравнивание рельефа кожи. По результатам УЗИ выявлены зоны различной акустической плотности, свидетельствующие о постепенном увеличении плотности соединительнотканных структур. Повышение эхогенности свидетельствовало о структурной перестройке и увеличении количества соединительнотканных компонентов. Гистологическое исследование биоптатов подтвердило активный процесс реструктуризации соединительной ткани. Сравнение с гистологической картиной, наблюдаемой до процедуры, установило формирование более плотной сети коллагеновых волокон на уровне дермы и подкожной жировой клетчатки.</p></sec><sec><title>Выводы</title><p>Выводы. Разработанный гибридный протокол характеризовался благоприятным профилем клинической эффективности и безопасности на фоне гистологически и сонографически подтвержденной структурной реорганизации соединительной ткани с накоплением коллагеновых волокон на уровнях дермы и подкожной жировой клетчатки.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Poly-L-lactic acid (PLLA) has a multifactorial effect on skin aging. It stimulates extracellular matrix components synthesis. High-molecular weight hyaluronic acid (HA) offers the advantage of maintaining optimal skin hydration and inhibiting matrix metalloproteinases. The combination of these agents appears to be promising, as evidenced by the synergistic effect observed in their actions.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the efficacy and safety of a hybrid one session treatment with PLLA and high-molecular weight HA for skin rejuvenation using both ultrasound (US) and histological diagnosis.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Reconstituted PLLA was injected to a depth of 3–4 mm into the anterior abdominal wall in six healthy females. Intradermal injections of HA were conducted to a depth of 2–3 mm. The efficacy of the injection treatments was assessed after 24 hours and 6 weeks based on visual assessment, photography, US, and histologic examination.</p></sec><sec><title>Results</title><p>Results. Six weeks after the treatment, the clinical signs included skin thickening and visual smoothing of the skin relief. Ultrasound results demonstrated zones of varying acoustic density, indicative of a gradual increase in connective tissue density. Areas of hyperechogenic density indicated structural reorganization and an increase in the number of connective tissue components. Histological examination of biopsy specimens confirmed active connective tissue restructuring. A comparison with the pre-treatment histology confirmed the formation of a denser network of collagen fibers at the level of dermis and subcutaneous adipose tissue.</p></sec><sec><title>Conclusions</title><p>Conclusions. The developed hybrid protocol was characterized by a favorable clinical efficacy and safety profile.
