Бронхолегочная дисплазия (БЛД) – это хроническое заболевание легких у детей, являющееся осложнением преждевременных родов, связанное с неблагоприятными неврологическими исходами, высокой частотой респираторной заболеваемости и смертностью. В настоящее время появился новый рекомендованный метод профилактики БЛД с помощью ингаляционного введения препарата легочного сурфактанта Сурфактант-БЛ (МНН таурактант).</p></sec><sec><title>Цель</title><p>Цель. Провести анализ динамики балльной оценки УЗ-картины легких в процессе ингаляционного введения СурфактантаБЛ, назначенного с целью профилактики развития БЛД.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование проведено на базе ГБУЗ СО «Екатеринбургский клинический перинатальный центр» в период с 27.01.2024 г. по 27.01.2025 г. Включено 14 детей со сроком гестации &lt; 30 нед., которые к 8–14-м сут. жизни получали респираторную терапию методами назального СРАР или ИВЛ.</p></sec><sec><title>Результаты</title><p>Результаты. В процессе проведенного нами исследования и анализа были отмечены случаи уменьшения интенсивности респираторной терапии, а именно: один ребенок экстубирован и переведен на назальный СРАР; двое детей, исходно находившихся на СРАР, были переведены на низкопоточные назальные канюли (НПК); один ребенок, исходно находившийся на СРАР, был переведен на подачу О2 свободным потоком. Сравнительный анализ таких показателей, как частота ИВЛ, параметры PiP, частота НПК, FiO2, а также балльная оценка по УЗИ легких до 1-й и после 5-й дозы Сурфактанта-БЛ, не показал статистически значимых различий в основной выборке исследования (р &gt; 0,05).</p></sec><sec><title>Выводы</title><p>Выводы. Ингаляционное применение Сурфактанта-БЛ (МНН таурактант) с целью профилактики БЛД у недоношенных новорожденных является новым и перспективным методом, вероятно снижающим риск и тяжесть респираторного повреждения. Однако в настоящее время имеются ограниченные данные по использованию данной методики, и для определения ее эффективности и безопасности требуется проведение дополнительных исследований.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Bronchopulmonary dysplasia (BPD) is a chronic lung disease in children, which is a complication of premature birth, associated with adverse neurological outcomes, high incidence of respiratory morbidity and mortality. Currently, a new recommended method for the prevention of BPD has appeared using inhalation administration of the surfactant drug Tauractant.</p></sec><sec><title>Aim</title><p>Aim. Аnalyze the dynamics of the scoring of the ultrasound picture of the lungs during the inhalation administration of Tauractant, prescribed for the prevention of BPD.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was conducted at the State Budgetary Healthcare Institution of the Sverdlovsk Region “Yekaterinburg Clinical Perinatal Center” in the period from 27.01.2024 to 27.01.2025. Included 14 children with a gestational age of &lt; 30 weeks, who received respiratory therapy by the methods of nasal CPAP or mechanical ventilation by the 8–14th day of life.</p></sec><sec><title>Results</title><p>Results. During our study and analysis, we noted cases of decreasing the intensity of respiratory therapy, namely: one child was extubated and transferred to nasal CPAP; two children initially on CPAP were transferred to low-flow nasal cannulas (LFNC); one child initially on CPAP was transferred to free-flow O2. Comparative analysis of such parameters as the frequency of mechanical ventilation, PiP parameters, LFNC frequency, FiO2, as well as the score according to lung ultrasound before the 1st and after the 5th dose of Tauractant did not show statistically significant differences in the main study sample (p &gt; 0,05).</p></sec><sec><title>Conclusion</title><p>Conclusion. Inhalation use of Tauractant for the prevention of BPD in premature infants is a new and promising method that probably reduces the risk and severity of respiratory injury.
