Кохлеарная имплантация (КИ) является самым эффективным средством реабилитации детей с глубокой потерей слуха. Выбор настроек процессора КИ является важной задачей, т. к. от параметров стимуляции процессора зависит разборчивость речи ребенка и ее понимание, а, следовательно, и последующее слухоречевое развитие и интеграция ребенка в общество.</p></sec><sec><title>Цель</title><p>Цель. Оценить слухоречевое развитие в группе детей от 3 до 14 лет, пользующихся различными системами КИ: Medel, Advanced Bionics, Cochlear, Nurotron, Oticon и Neurelec.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование было включено 82 ребенка, использующих КИ. Проведен сбор жалоб, анамнеза, осмотр лор-органов, консультация сурдопедагога, настройка процессора КИ, оценка разборчивости речи методом речевой аудиометрии в свободном звуковом поле (n = 82) в процессе настройки процессора КИ и сразу после настройки. В группе 56 детей (n = 56) дошкольного возраста от 5 до 7 лет провели анкетирование родителей при помощи шкалы MUSS (Meaningful Use of Speech Scale – шкала значимого использования речи).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. По данным речевой аудиометрии в свободном звуковом поле разборчивость односложных слов составила 70,85 ± 1,44%, а разносложных слов – 74,51 ± 1,60%. Результаты разборчивости речи после проведенной настройки процессора КИ достоверно улучшились и составили: односложных слов – 76,95 ± 1,31% (p &lt; 0,01), а разносложных – 83,78 ± 1,43% (p &lt; 0,001).</p></sec><sec><title>Выводы</title><p>Выводы. Шкала значимого использования речи (MUSS) является простым инструментом оценки развития устной речи у имплантированных детей дошкольного возраста. Речевая аудиометрия в свободном звуковом поле может использоваться как для оценки уровня разборчивости речи, так и для определения параметров стимуляции речевого процессора.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cochlear implantation (CI) is the most effective way of rehabilitation for children with profound hearing loss. Choosing the settings of the CI processor is an important task, because the child’s speech intelligibility and understanding depend on the parameters of processor stimulation, and, consequently, the subsequent auditory-speech development and integration of the child into society.</p></sec><sec><title>Aim</title><p>Aim. To evaluate auditory and speech development in a group of children aged from 3 to 14 years using different CI systems: Medel, Advanced Bionics, Cochlear, Nurotron, Oticon and Neurelec by using the MUSS Scale (Meaningful Use of Speech Scale) and the method of speech audiometry in a free field sound.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study includes 82 children with CI. The children received complaints, medical history, examination of ENT organs, consultation with a surdo teacher, setting of the CI processor, assessment of speech intelligibility by using speech audiometry in a free field (n = 82) during the process of setting the CI processor and after setting. In a group of 56 children (n = 56) of preschool age from 5 to 7 years, parents answered the questions by using the MUSS Scale (Meaningful Use of Speech Scale).</p></sec><sec><title>Results</title><p>Results. According to speech audiometry data, the intelligibility of monosyllabic words in the free field was 70.85 ± 1.44%, and of multisyllabic words – 74.51 ± 1.60%. The results of speech intelligibility after setting the CI processor significantly improved and amounted to: monosyllabic words – 76.95 ± 1.31% (p &lt; 0.01), and multisyllabic words – 83.78 ± 1.43% (p &lt; 0.001).</p></sec><sec><title>Conclusion</title><p>Conclusion. The Scale of Meaningful Speech Use (MUSS) is a simple tool for assessing the development of speech in cochlear implanted preschool children.
