Эпидемия коронавирусной инфекции, которая началась в 2019 г. и быстро распространилась по всему миру, характеризовалась вирусной пневмонией и острой гипоксемической дыхательной недостаточностью.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность применения неинвазивной вентиляции легких у пациентов с острой дыхательной недостаточностью на фоне коронавирусной инфекции на этапе, предшествующем переводу в отделение реанимации.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В ретроспективное когортное исследование были включены пациенты с подтвержденным диагнозом коронавирусной инфекции и острой дыхательной недостаточностью. Для анализа демографические, клинические и лабораторные данные собирались при поступлении, а показатели дыхательной функции фиксировались как до начала неинвазивной вентиляция легких, так и в первые сутки ее применения.</p></sec><sec><title>Результаты</title><p>Результаты. У большинства пациентов неинвазивная вентиляция легких оказалась успешной, однако часть пациентов потребовала перевода в отделение реанимации из-за ее неэффективности. Общая смертность в исследуемой группе была значительной. Пациенты, у которых неинвазивная вентиляция легких не дала результата, были старше, имели более высокую частоту дыхания и уровни некоторых биохимических маркеров при поступлении. Через несколько часов после начала неинвазивной вентиляции легких у этих пациентов наблюдались более высокие показатели минутной вентиляции и вентиляционного коэффициента. Выявлены факторы, которые могут служить предикторами неэффективности неинвазивной вентиляции легких, включая низкую насыщенность кислородом до начала вентиляции, повышенные значения вентиляционного коэффициента, минутной вентиляции и дыхательного объема через несколько часов после начала вмешательства.</p></sec><sec><title>Выводы</title><p>Выводы. Таким образом, неинвазивная вентиляция легких может быть эффективным методом лечения пациентов с острой гипоксической дыхательной недостаточностью на фоне коронавирусной инфекции на дореанимационном этапе.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The coronavirus epidemic, which began in 2019 and quickly spread throughout the world, was characterized by viral pneumonia and acute hypoxemic respiratory failure.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the effectiveness of non-invasive ventilation in patients with acute respiratory failure due to coronavirus infection at the pre-intensive care unit stage.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective cohort study included patients with confirmed coronavirus infection and acute respiratory failure. Demographic, clinical, and laboratory data were collected at admission, while respiratory parameters were recorded both before the initiation of noninvasive ventilation and during the first 24 hours of its application.</p></sec><sec><title>Results</title><p>Results. For most patients, noninvasive ventilation was successful, but some required transfer to the intensive care unit due to its ineffectiveness. Overall mortality in the study group was significant. Patients for whom non-invasive ventilation was ineffective were older and had higher respiratory rates and levels of certain biochemical markers upon admission. А few hours after the initiation of noninvasive ventilation, these patients demonstrated higher minute ventilation and ventilatory ratio values. Factors identified as potential predictors of noninvasive ventilation failure included low oxygen saturation before ventilation, elevated ventilatory ratio, increased minute ventilation, and tidal volume several hours after the intervention began.</p></sec><sec><title>Conclusion</title><p>Conclusion.
