Миокардиальные мостики (ММ) являются самой частой врожденной аномалией развития коронарных артерий, преимущественно затрагивая переднюю межжелудочковую артерию (ПМЖА).Цель. Разработать критерии ранней диагностики нарушения перфузии миокарда у бессимптомных пациентов с ММ в области ПМЖА.Материалы и методы. Исследование включало несколько этапов. На первом этапе для выявления пациентов с ММ в области ПМЖА и оценки их анатомо-топографических особенностей был проведен анализ 811 заключений компьютернотомографической (КТ) коронарографии. На втором этапе для изучения клинического значения ММ были проанализированы 20 историй болезни пациентов с ММ в области ПМЖА (основная группа) и 20 историй болезни пациентов без ММ (группа сравнения). На третьем этапе были обследованы 20 пациентов с бессимптомными ММ в области ПМЖА (основная группа) и 20 без ММ (группа сравнения), которым проводилась КТ-перфузия миокарда и speckle tracking эхокардиография (СТ-ЭхоКГ) для оценки их эффективности в ранней диагностике нарушения кровоснабжения миокарда. На основании полученных результатов разрабатывались критерии раннего нарушения перфузии миокарда у бессимптомных пациентов с ММ в области ПМЖА.Результаты. Клинико-лабораторные показатели (мужской пол, возраст &lt; 60 лет, нарушения липидного обмена) и инструментальные данные: 1) суточное мониторирование электрокардиограммы: наджелудочковые и желудочковые нарушения сердечного ритма, а также безболевая ишемия миокарда (более 1 мин); 2) тредмил-тест (велоэргометрия): депрессия или элевация сегмента ST в отведениях V1–2, тредмил-индекс Дюка ≤ -1 балла; 3) ЭхоКГ: E/А ≤ 1, е’mean ≤ 8,5 см/сек, Е/е’mean ≥ 9; 4) СТ-ЭхоКГ: глобальная продольная пиковая деформация левого желудочка ≤ -20,55%.Выводы. Разработанные критерии позволяют заподозрить наличие нарушения перфузии миокарда у бессимптомных пациентов с ММ в области ПМЖА, для подтверждения которого необходимо выполнение визуализирующих методов исследования, в частности КТ-коронарографии, дополненной оценкой перфузии миокарда.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Myocardial bridges (MBs) are the most common congenital anomaly of coronary arteries development, mainly affecting the left anterior descending (LAD) artery.Аim. To develop criteria for early diagnosis of myocardial perfusion disorders in asymptomatic patients with MBs of the LAD artery.Materials and methods. The study was conducted in several stages. At the first stage, 811 coronary computed tomography (CT) angiography reports were analyzed to identify patients with MBs of the LAD artery and to assess their anatomical and topographic features. At the second stage, 20 medical histories of patients with MBs of the LAD artery (main group) and 20 medical histories of patients without MBs (comparison group) were analyzed to assess the clinical significance of MBs. At the third stage, 20 patients with asymptomatic MBs of the LAD artery (main group) and 20 patients without MBs (comparison group) were examined, who underwent stress CT myocardial perfusion and speckle tracking echocardiography (STE) to assess the effectiveness of these methods of the early diagnosis of myocardial perfusion disorders in patients with MBs. Based on the obtained results, criteria for early myocardial perfusion disorders in asymptomatic patients with MBs of the LAD artery were developed.Results. Clinical and laboratory parameters: (male gender, age &lt; 60 years, dyslipidemia) and instrumental data: 1) 24-hour Holter monitoring: supraventricular and ventricular heart arrhythmias, silent myocardial ischemia (more than 1 min); 2) treadmill test (bicycle ergometry): ST segment depression or elevation in leads V1–2, the Duke Treadmill Score ≤ -1 point; 3) echocardiography: E/A ≤ 1, e’mean ≤ 8.5 cm/sec, E/e’mean ≥ 9; 4) STE: global longitudinal peak strain of the left ventricle ≤ -20.55%.Conclusions.
