Воспалительные заболевания кишечника (ВЗК), включающие болезнь Крона (БК) и язвенный колит (ЯК), представляют собой хронические рецидивирующие состояния с поражением слизистой оболочки желудочно-кишечного тракта и возможными местными и системными осложнениями. Частота встречаемости остеопороза (ОП) у пациентов с ВЗК, по данным различных исследований, очень вариабельна.</p></sec><sec><title>Цель</title><p>Цель. Изучить изменение минеральной плотности костной ткани (МПКТ) и параметров костного ремоделирования у пациентов молодого возраста с ВЗК для ранней диагностики изменений костной ткани.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 123 пациента с верифицированными диагнозами ЯК (n = 70) и БК (n = 53), наблюдавшихся в ГАУЗ «РКБ МЗ РТ» c марта 2023 г. по декабрь 2024 г. Всем пациентам проведен сбор анамнеза и тщательное физикальное обследование. Оценивались маркеры костного метаболизма и его регуляторов: остеокальцин (нг/мл), Beta-Cross Laps (нг/мл), P1NP (нг/мл), оценка уровня витамина D (нг/мл). Для диагностики ОП всем была выполнена оценка МПКТ с помощью двухэнергетической рентгеновской абсорбциометрии.</p></sec><sec><title>Результаты</title><p>Результаты. Из 123 пациентов молодого возраста с ВЗК у 13,8% (17 пациентов) уже было выявлено снижение костной массы ниже возрастной нормы по данным показателей двухэнергетической рентгеновской абсорбциометрии. При оценке параметров костного ремоделирования были выявлены изменения у 31% (39 пациентов), уровень витамина D снижен у всех пациентов в исследовании.</p></sec><sec><title>Выводы</title><p>Выводы. Среди 123 обследованных молодых пациентов с ВЗК у 13,8% (17) выявлено снижение костной массы ниже возрастной нормы, причем параметры костного ремоделирования были изменены у большего числа пациентов (31%), витамин D снижен у всех пациентов с ВЗК. На уменьшение Z-критерия, а соответственно, и на снижение МПКТ достоверно влияют наличие умеренной и выраженной активности ВЗК, низкий индекс массы тела, наличие системных глюкокортикоидов в протоколе лечения ВЗК, а также острое течение ЯК и наличие илеоколита при БК.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Inflammatory bowel diseases (IBD) including Crohn's disease (CD) and ulcerative colitis (UC) are chronic relapsing conditions with gastrointestinal mucosa injuries and potential local and systemic complications. The incidence of osteoporosis (OP) in patients with IBD is very variable according to data from different studies.</p></sec><sec><title>Aim</title><p>Aim. To evaluate changes in bone mineral density (BMD) and bone remodelling parameters in young patients with IBD for early detection of changes in the bone tissue.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 123 patients with verified diagnoses of UC (n = 70) and CD (n = 53), who were observed at the State Autonomous Healthcare Institution Republican Clinical Hospital of the Ministry of Health of the Republic of Tatarstan from March 2023 to December 2024. All patients underwent a detailed clinical history and physical examination. Bone metabolism and its regulators such as osteocalcin (ng/ml), Beta-Cross Laps (ng/ml), P1NP (ng/ml), and vitamin D levels (ng/ml) were assessed. All patients underwent measurements of BMD using dual-energy X-ray absorptiometry to detect OP.</p></sec><sec><title>Results</title><p>Results. Of 123 young patients with IBD, 13.8% (17 patients) had bone mass below age-matched reference values based on dual-energy X-ray absorptiometry findings. Bone remodelling parameters assessment revealed changes in 31% (39 patients), and decrease in vitamin D levels in all patients of the study.</p></sec><sec><title>Conclusions</title><p>Conclusions. Of 123 young patients with IBD examined, 13.8% (17) of patients had bone mass below age-matched reference values. The bone remodelling parameters were changed in a larger percentage of patients (31%). All patients with IBD had low vitamin D levels.
