Увеальная меланома относится к редким злокачественным новообразованиям, формирующимся из меланоцитов сосудистой оболочки глазного яблока. В структуре внутриглазных злокачественных новообразований она занимает лидирующую позицию.</p></sec><sec><title>Цель</title><p>Цель. Проанализировать данные реальной клинической практики по использованию ниволумаба в комбинации с ипилимумабом для лечения пациентов с метастатической увеальной меланомой.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Пятнадцать больных с метастатической увеальной меланомой получали терапию препаратами ниволумаб и ипилимумаб. Медиана возраста пациентов составила 53 года, минимальный возраст – 24 года, а максимальный – 71 год. В анализ включены 9 (60%) мужчин и 6 (40%) женщин. Оценка функционального состояния показала, что 4 (26,7%)</p><p>пациента имели ECOG 0 баллов, 6 (40%) – ECOG 1 и 5 (33,3%) – ECOG 2.</p></sec><sec><title>Результаты</title><p>Результаты. Медиана безрецидивной выживаемости после энуклеации глаза составила 61 мес. Объективный ответ на иммунотерапии достигнут у 33,3% пациентов, а частота контроля заболевания составила 86,6%. Однако 4 (26,6%) пациента прекратили терапию из-за гепатотоксичности. В ходе исследования отмечено, что медиана общей выживаемости от начала терапии ниволумабом в комбинации с ипилимумабом достигает 8 мес. Установлено, что функциональный статус пациентов и ответ на терапию являются важными предикторами выживаемости, где у пациентов с лучшими показателями продолжительность жизни значительно выше. Таким образом, пациенты с ECOG 0 показывают медиану общей выживаемости в 18 мес.</p></sec><sec><title>Выводы</title><p>Выводы. Ингибиторы контрольных точек иммунного ответа остаются основным методом лечения пациентов с метастатической увеальной меланомой. Комбинированная иммунотерапия ниволумабом и ипилимумабом может рассматриваться как возможный вариант терапии данной категории пациентов в условиях ограниченных терапевтических возможностей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Uveal melanoma is a rare malignant neoplasm that develops from melanocytes of the choroid of the eyeball. It occupies a leading position in the structure of intraocular malignant neoplasms.</p></sec><sec><title>Aim</title><p>Aim. Тo analyze real-world clinical practice data on the use of nivolumab in combination with ipilimumab to treat patients with metastatic uveal melanoma.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Fifteen patients with metastatic uveal melanoma received therapy with nivolumab and ipilimumab. The median age of patients was 53 years, the minimum age was 24 years, and the maximum was 71 years. The analysis included 9 (60%) men and 6 (40%) women. Evaluation of the functional status showed that 4 (26.7%) patients had ECOG 0 points, 6 (40%) – ECOG 1 and 5 (33.3%) – ECOG 2.</p></sec><sec><title>Results</title><p>Results. The median relapse-free survival after enucleation of the eye was 61 months. Objective response to immunotherapy was achieved in 33.3% of patients, and the disease control rate was 86.6%. However, 4 (26.6%) patients discontinued therapy due to hepatotoxicity. The study noted that the median overall survival from the start of therapy with nivolumab in combination with ipilimumab reaches 8 months. It was found that the functional status of patients and the response to therapy are important predictors of survival, where patients with better indicators have a significantly higher life expectancy. Thus, patients with ECOG 0 show a median overall survival of 18 months.</p></sec><sec><title>Conclusion</title><p>Conclusion. Immune checkpoint inhibitors remain the mainstay of treatment for patients with metastatic uveal melanoma.
