Ингибиторы ABL тирозинкиназы (ИТК) существенно улучшили результаты терапии пациентов с хроническим миелоидным лейкозом (ХМЛ). Бозутиниб является ИТК второго поколения, который показал высокую эффективность и безопасность в клинических исследованиях как в первой, так и в последующих линиях терапии ХМЛ.</p></sec><sec><title>Цель</title><p>Цель. Оценить долгосрочную эффективность и безопасность бозутиниба в реальной клинической практике.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В данное ретроспективное исследование включены все пациенты в первой хронической фазе ХМЛ, когда-либо получавшие терапию препаратом бозутиниб в качестве 1-го, 2-го или 3-го ИТК. В качестве 1-го ИТК бозутиниб принимали всего 4 пациента (мужчин – 2) в возрасте от 30 до 41 года. Бозутиниб после неудачи терапии иматинибом или 2 ИТК получали, соответственно, 19 пациентов (мужчин – 12) с медианой возраста 50 (31–71) и 17 пациентов (мужчин – 8)  с медианой возраста 49,8 (21–70) на момент начала приема препарата. Большинство пациентов, получавших бозутиниб как 2-й или 3-й ИТК, имели длительный анамнез ХМЛ и большинство из них были резистентны к предшествующим ИТК.</p></sec><sec><title>Результаты</title><p>Результаты. В группе пациентов, получавших бозутиниб в первой линии терапии ХМЛ, все четверо достигли полного гематологического ответа (ПГО), а полный цитогенетический ответ (ПЦО) и большой молекулярный ответ (БМО) получили трое (75%) из них. Все трое продолжали терапию более 16 лет без значимых побочных эффектов. При назначении бозутиниба после неудачи терапии только иматинибом или 2 ИТК, ПГО был достигнут у большинства пациентов, а большой цитогенетический ответ (БЦО) – в 7/19 (36,8%) и 6/17 (35,3%) случаях. За время наблюдения медиана общей выживаемости не была достигнута. Случаев отмены бозутиниба из-за непереносимости препарата было мало. Длительный прием препарата не приводил к серьезным отсроченным побочным эффектам, в том числе со стороны сердца и сосудов, связанным с терапией бозутинибом.</p></sec><sec><title>Выводы</title><p>Выводы. Таким образом, в нашей группе пациентов при длительном наблюдении бозутиниб показал не только высокую частоту достижения оптимального ответа во всех линиях терапии в хронической фазе ХМЛ, но и отсутствие тяжелых отдаленных осложнений как гематологических, так и негематологических.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. ABL tyrosine kinase inhibitors (TKIs) have significantly improved the treatment outcomes for patients with chronic myeloid leukemia (CML). Bosutinib is a second-generation TKI that has demonstrated high efficacy and safety in clinical trials in firstand subsequent-line therapy of CML.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the long-term efficacy and safety of bosutinib in real-world clinical practice.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. In our retrospective study all patients in the first chronic phase of CML who had ever received bosutinib therapy as their first, second, or third TKI were included. Only 4 patients (2 men) aged from 30 to 41 years received bosutinib as first TKI. Bosutinib was administered as second or third TKI to 19 patients (12 men) with a median age of 50 (31–71) and 17 patients (8 men) with a median age of 49.8 (21–70), respectively. Most patients had a long history of CML and most of them were resistant to previous TKIs.</p></sec><sec><title>Results</title><p>Results. All four patients who received bosutinib in the first-line achieved a complete hematological response (CHR), and three (75%) of them achieved a complete cytogenetic response (CCR) and a major molecular response (MMR). All 3 patients with MMR continued therapy for more than 16 years without significant adverse events. Among patients with bosutinib treatment after failure of imatinib or 2 TKIs, the CHR was achieved in most cases, and major cytogenetic response (MCR) was achieved in 7/19 (36.8%) and 6/17 (35.3%) cases. During the observation period, the median overall survival was not reached. There were few cases of bosutinib discontinuation due to drug intolerance. Long-term use of the drug did not lead to serious delayed side effects, including cardiovascular complications, associated with bosutinib therapy.</p></sec><sec><title>Conclusions</title><p>Conclusions.
