Бесплодие представляет собой значимую глобальную проблему здравоохранения, затрагивающую миллионы людей репродуктивного возраста. Согласно данным Всемирной организации здравоoхранения (ВОЗ), около 17,5% взрослого населения страдает от бесплодия, что соответствует примерно каждому шестому человеку в мире. Влияние бактериальных сообществ эндометрия на репродуктивную функцию человека остается предметом активного обсуждения в научном сообществе. Существующие данные свидетельствуют о том, что состав микробиоты эндометрия может оказывать как положительное, так и отрицательное воздействие на фертильность, однако механизмы этих взаимодействий требуют дальнейшего изучения.</p></sec><sec><title>Цель</title><p>Цель. Оценить особенности состава эндометриоидного микробиома у женщин с бесплодием и повторными неудачными программами ЭКО и его влияние на исходы циклов вспомогательных репродуктивных технологий (ВРТ).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено пилотное проспективное одноцентровое нерандомизированное исследование, включившее 53 пациентки возраста от 18 до 42 лет с бесплодием и неудачными попытками ЭКО в анамнезе. Дополнительно выполнена биопсия эндометрия с 15-го по 25-й день менструального цикла с целью оценки микробиома эндометрия методом метагеномного анализа (MEMA).</p></sec><sec><title>Результаты</title><p>Результаты. По результатам метагеномного секвенирования эндометрия 19 пациенток имели преобладание Lactobacillus spp., у 23 другие штаммы, у 11 дисбиотические нарушения. Клиническая беременность была зафиксирована у 7 (30,4%) из 23 пациенток с условно-патогенными микроорганизмами и у 8 (72,7%) из 11 женщин с дисбиотическими нарушениями на фоне проведенной терапии.</p></sec><sec><title>Выводы</title><p>Выводы. Резюмируется, что у женщин, не нуждающихся в лечении, вероятность наступления беременности в 4,75 раза выше, чем у женщин, получающих терапию (p = 0,021). При этом шансы наступления беременности у пациенток, получавших антибиотикотерапию с последующей пробиотикотерапией, были в 7,7 раза ниже по сравнению с пациентками, не получавшими данное лечение.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Infertility is a significant global health problem affecting millions of people of reproductive age. According to the World Health Organization (WHO), approximately 17.5% of the adult population suffers from infertility, which corresponds to about one in six people worldwide. The impact of endometrial bacterial communities on human reproductive function remains a subject of active discussion in the scientific community. Current evidence suggests that the composition of the endometrial microbiota may have both positive and negative effects on fertility, but the mechanisms of these interactions require further investigation.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the features of endometrioid microbiome composition in women with infertility and repeated failed IVF programs and its impact on the outcomes of assisted reproductive technology cycles.</p></sec><sec><title>Materials and metods</title><p>Materials and metods. A pilot prospective single-center non-randomized study including 53 female patients aged from 18 to 42 years with infertility and unsuccessful IVF attempts in anamnesis was conducted. Additionally, endometrial biopsy was performed from the 15th to the 25th day of the menstrual cycle in order to assess the endometrial microbiome by metagenomic analysis (MEMA).</p></sec><sec><title>Results</title><p>Results. According to the results of endometrial metagenomic sequencing, 19 patients had a predominance of Lactobacillus spp., 23 had other strains, and 11 had dysbiotic disorders. Clinical pregnancy was recorded in 7 (30.4%) of the 23 patients with opportunistic microorganisms and in 8 (72.7%) of the 11 women with dysbiotic disorders after the treatment.</p></sec><sec><title>Conclusions</title><p>Conclusions. It is summarized that women who do not need treatment are 4.75 times more likely to become pregnant than women who receive therapy (p = 0.021).
