Повышение эффективности консервативного лечения экссудативного среднего отита (ЭСО) у детей остается актуальной проблемой ввиду полиэтиологичности данного заболевания, а также частого использования хирургической тактики разрешения секреторного отита. Статья объединяет кафедральный опыт наблюдения за течением ЭСО после ОРИ верхних дыхательных путей при различных видах терапии, направленной на разные звенья патогенеза заболевания.</p></sec><sec><title>Цель</title><p>Цель. Оценить клиническую эффективность лечения ЭСО у детей, перенесших ОРИ, интраназальным введением азоксимера бромида в сравнении с комплексной терапией мукорегулятором, топическими антибактериальными средствами и деконгестантом.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено клиническое рандомизированное простое открытое проспективное сравнительное исследование с участием 49 детей в возрасте 4–7 лет с ЭСО после ОРИ. Группа наблюдения – 23 пациента, которые получали монотерапию азоксимера бромида (раствор) интраназально по 3 капли в каждый носовой ход 3 раза в день в течение 2 нед. Группа контроля – 26 пациентов, которые получали комплексную терапию (интраназально комбинированный спрей неомицина и полимиксина, дексаметазона метабензоата и фенилэфрина по одному впрыскиванию в каждую ноздрю 3 раза в день в течение 10 дней) и пероральный прием карбоцистеина в дозе, соответствующей возрасту, в течение 2 нед.</p></sec><sec><title>Результаты</title><p>Результаты. На фоне терапии азоксимера бромидом динамика восстановления интратимпанального давления не уступала таковой при стандартном лечении, при этом число рецидивов ЭСО в последующие 3 и 6 мес. было меньше.</p></sec><sec><title>Выводы</title><p>Выводы. Возрастные особенности местного иммунитета, характерные для детей 4–7 лет могут быть предрасполагающим фактором вялотекущего катарального воспаления слизистой оболочки слуховой трубы и барабанной полости после ОРИ. Интраназальная иммунокоррекция азоксимера бромидом купирует катаральный воспалительный процесс барабанной полости и слуховой трубы при развитии ЭСО после ОРВИ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Improving the effectiveness of conservative treatment of secretory otitis media (SOM) in children remains an urgent problem because of the multifactorial etiology of the disease and the continued reliance on surgical intervention. The article presents the collective experience of monitoring the course of SOM after ARI with therapy aimed at various links in the pathogenesis of the disease.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the clinical efficacy of intranasal azoximer bromide (Polyoxidonium ® , Petrovax, Russia) versus combination therapy with a mucoregulator, topical antibacterial agents, and a decongestant for the treatment of secretory otitis media in children after acute respiratory viral infection.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A randomized, open-label, prospective, comparative clinical study was conducted in 49 children aged 4–7 years with SOM after ARI. The study group included 23 patients who received intranasal azoximer bromide, 3 drops in each nostril three times daily for two weeks. The control group comprised 26 patients who received combination therapy: an intranasal spray containing neomycin, polymyxin, dexamethasone metabenzoate, and phenylephrine (one administration per nostril three times daily for 10 days) plus oral carbocysteine at an age-appropriate dose for two weeks.</p></sec><sec><title>Results</title><p>Results. During azoximer bromide treatment, the dynamics of intratympanic pressure recovery were not inferior to those with standard therapy, and SOM relapses were less frequent at 3 and 6 months.</p></sec><sec><title>Conclusions</title><p>Conclusions. Age-related characteristics of local immunity in children aged 4-7 years may underlie chronic catarrhal inflammation of the mucous membrane of the auditory tube and tympanic cavity after acute respiratory viral infection.
