Современные стратегии лечения псориаза направлены на облегчение симптомов, улучшение качества жизни и предотвращение прогрессирования заболевания. В случае отсутствия положительного эффекта от проводимой стартовой терапии рассматривается вопрос о применении различных биологических или низкомолекулярных препаратов, воздействующих на определенные иммунные механизмы, лежащие в основе патогенеза заболевания. Современные терапевтические стратегии направлены на разработку новых препаратов, которые бы блокировали передачу сигналов эффекторных цитокинов. К настоящему времени отсутствуют надежные предикторы, позволяющие предсказать, какой пациент положительно отреагирует на тот или иной вид системной терапии. Оказать помощь в ответах на эти вопросы способен поиск биомаркеров, участвующих в патогенезе заболевания и/или отражающих механизм действия лекарственных средств. В представленной статье будут рассмотрены предиктивные иммунологические биомаркеры эффективности лечения псориаза биологическими препаратами (ингибиторами TNF-α, ингибиторами IL-17, ингибиторами IL-23, ингибиторами фосфодиэстеразы 4-го типа), в качестве которых могут выступать цитокины, хемокины, адипокины, растворимые формы рецепторов клеточной поверхности, маркеры иммунной активации. В нашей статье мы не рассматриваем новые терапевтические стратегии при псориазе, направленные на малые молекулы, которые воздействуют на сигнальные пути JAK-STAT (ингибиторы янус-киназ, ингибиторы тирозинкиназы-2, ингибиторы ретиноидного орфанного ядерного рецептора-γt), предотвращающие передачу сигналов ниже IL-23 и IL-6. Учитывая то, что в патогенез развития псориаза вовлечено множество молекулярно-клеточных механизмов иммунного ответа, представляется перспективным поиск комбинаций данных маркеров и составление соответствующих моделей прогнозирования эффективного ответа на терапию на их основе.</p></abstract><trans-abstract xml:lang="en"><p>Psoriasis is one of the most common systemic chronic immune-mediated inflammatory diseases. Current treatment strategies for psoriasis focus on relieving symptoms, improving quality of life, and preventing disease progression. In the absence of a positive effect from the initial therapy, the issue of using various biological or low molecular weight drugs that affect certain immune mechanisms underlying the pathogenesis of the disease is considered. Current therapeutic strategies aim to develop new drugs that would block effector cytokine signaling. To date, there are no reliable predictors to predict which patient will respond positively to a particular type of systemic therapy. The search for biomarkers involved in the pathogenesis of the disease and/or reflecting the mechanism of action of drugs can help in answering these questions. The presented article will consider predictive immunological biomarkers of the effectiveness of psoriasis treatment with biological drugs (TNF-α inhibitors, IL-17 inhibitors, IL-23 inhibitors, type 4 phosphodiesterase inhibitors), which can be cytokines, chemokines, adipokines, soluble forms of cell surface receptors, markers of immune activation. In our paper, we do not consider new therapeutic strategies in psoriasis targeting small molecules that target JAK-STAT signaling pathways (Janus kinase inhibitors, tyrosine kinase 2 inhibitors, retinoid orphan nuclear receptor γt) preventing signaling downstream of IL-23 and IL-6.
